We are seeking a dedicated Nurse Care Manager to join our integrated outpatient team and help bridge the gap between physical and behavioral healthcare.
In this rewarding role, you'll work alongside behavioral health professionals, primary care providers, hospitals, residential programs, and community partners to ensure individuals receive coordinated, high-quality care that supports their overall health and wellness.
What You'll Do
As a Nurse Care Manager, you will:
- Collaborate with multidisciplinary teams to coordinate medical and behavioral healthcare services.
- Support individuals transitioning from hospitals, crisis services, residential programs, and other treatment settings.
- Conduct ongoing assessments of medical and psychiatric needs through chart reviews, client contacts, and provider collaboration.
- Coordinate care with primary care providers, specialists, pharmacies, hospitals, and community resources.
- Review medical records, laboratory results, medications, and treatment recommendations to promote continuity of care.
- Provide education and support to clients, families, caregivers, and staff on wellness, medications, symptom management, and preventive healthcare.
- Develop and facilitate health and wellness groups.
- Participate in quality improvement initiatives and agency-wide integrated care efforts.
- Provide nursing consultation and support to residential and crisis programs through the agency's nursing on-call rotation.

Schedule
- Full-time, exempt position
- Monday through Friday, generally 8:00 a.m. – 5:00 p.m.
- Participation in the agency's nursing on-call rotation required.
- Travel throughout the service area as needed.

· Bachelor of Science in Nursing is required.
· Active licensure as a Registered Nurse (RN) in the state of Maine is required.
· Minimum of two (2) years of nursing experience preferred, with experience in behavioral health, community mental health, substance use treatment, primary care, care management, or integrated healthcare settings strongly preferred.
· Valid driver’s license, reliable transportation, and proof of automobile insurance required, with the ability to travel within the service area as needed.
