What You’ll Do
As a Physical Therapist in a Skilled Nursing Facility, you will help patients regain mobility and independence following illness, injury, or surgery.
Key Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide evidence-based physical therapy interventions
- Collaborate with physicians, nurses, and rehab team members
- Supervise Physical Therapy Assistants (PTAs)
- Document patient progress accurately and timely
- Participate in care planning and interdisciplinary meetings
- Educate patients and caregivers on mobility and safety
- Support discharge planning and functional outcomes

Qualifications
- Graduate of an accredited Physical Therapy program (DPT, MS, or BS)
- Active Ohio Physical Therapist license (or eligible)
- Strong communication and teamwork skills
- Passion for working with geriatric and post-acute populations
