Hoag At Home Health services aim to provide the highest quality health care to the communities we serve. By offering at-home care, we are able to bring a patient’s continuum of the care-full circle. We help people recover from illness, injury, or surgery, avoid unnecessary rehospitalization and manage long-term conditions with nursing, therapy and other services at home. Position Details:

· Schedule:  Fulltime. Monday – Friday. No weekends or after hours

· Locations:  Southern Orange County 

· Setting: Nursing facilities, patient homes

· Call: Occasionally cover patient care needs after-hours as needed

· EMR:  Epic

· Staff: Medical Assistant, nurses, administrative support

Requirements:

· A Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited institution 

· Fellowship training completion in Hospice/Palliative Medicine 

· Board Certified in Internal Medicine 

· Board Certified in Hospice/Palliative Medicine

· 3-5 years of experience practicing in a home health setting is preferred

· Active California medical license (Or ability to obtain)
