RN Nurse Case Manager
Part-Time: 20 hours weekly
Pay Rate: $40 per hour
Location: Fitchburg/Gardner area
Bonus: $500 part-time
RN RESIDENTAL CASE MANAGER
SUMMARY OF POSITION:
The RN Residential Case Manager reports to the Director of Nursing. The RN will assist in the care coordination for the individuals served within the organization. This includes attend visits to the Health Care Provider (HCP), establish plans of care and facilitate health education and training to persons served, caregivers and/or residential staff in order to ensure the health and safety of the people served. The RN must have a general understanding of MAP and work a flexible schedule adapted to meet the needs of the agency. This is a Part-Time hourly position.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Oversee a shared caseload with the LPN Case Managers.
- Have a general understanding of MAP regulations.
- Conduct regular visits to residences to identify and provide plans of care for individuals.
- Identify areas of non-compliance and report to the Residential House Manager and Program Director and work with staff to achieve MAP compliance.
- Review and collate medical information through encounter forms, physical exams, and case studies provided by the Program Directors in both internal and external data management systems.
- Attend specific medical appointments for individuals as needed.
- Maintain confidentiality in keeping with HIPPA regulations and positive demeanor while successfully juggling multiple priorities simultaneously.
- Review and document on nursing plans of care monthly, after each hospitalization, or as needed.
- Assist in discharge instructions, medical treatments or medication changes to the Residential House Manager and all staff.
- Provide required health and nursing related training and education to staff on an annual and as needed basis.
- Identify safety risks and possible alternatives and advocate for the rights of all individuals.
- Handle related job duties in compliance with agency’s Programmatic Policy Manual.
- Be flexible and able to prioritize nursing and medical needs on a day by day basis.
- Varies duties as assigned.
- Travel as needed within 100 miles of main Sturbridge office.
MARGINAL DUTIES AND RESPONSIBILITIES:
- Ensure programs maintain the standards outlined in the Department of Developmental Services mission statement, the Survey and Certification Tool, as well as requirements of Performance Based Objectives.
- Attend and/or participate in training of the residential staff during their staff meetings.
- Attend monthly nursing team meetings.
- Implement the agency mission through service delivery, staff development, and resource acquisition.
- Provide recommendations to your supervisor regarding ways to improve the current systems
- Observe all Agency policies and procedures
- Accept responsibility and accountability as part of the position
- Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously.
- Maintain CPR, AED & Safety Care certifications.
PHYSICAL AND MENTAL REQUIREMENTS:
- Complete Agency Nursing Orientation
- Handle all projects with a positive attitude
- Ability to establish rapport with the disabled population, their families, co-workers and service providers.
· Organized, dependable and even temperament; good phone manner.
· Effective verbal and written communication skills
· Strong interpersonal and communication skills, and the ability to work with a wide range of employees.
· Must be able to bend, lift, stoop, reach and file.
MINIMUM ACCEPTABLE QUALIFICATIONS:
- Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
- At least one year of full time working experience as an LPN.
- Must be 21+ years old, have a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.
Employee must possess the knowledge; skills and abilities listed above and demonstrate that they can perform the essential functions of the job. Employee must be able to speak, read and write in fluent English.
• Licensed as a Registered Nurse in Massachusetts by the Board of Registration.
• Two years of experience as a registered nurse.
• Experience with the developmentally disabled population preferred.
• Must be 21+ years old, have a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.
