Department: Case Management

Grade Range/Job Status: Ful Time/Non-Exempt - Hourly

Reporting Relationship: Lead Case Manager

Supervisory Responsibilities: NA

Typical Schedule: Primarily M-F Days. Must occasionally be willing and able to work flexible hours/days, reflective of the dynamic schedule of the organization.

Position Summary

The Case Aide provides clerical and administrative support to the Case Management and Care Coordination departments. This position helps ensure accurate documentation, timely completion of administrative tasks, regulatory compliance, and efficient department operations. Hammer Case Management provides services contracted with Hennepin County for people receiving disability services through CADI, BI, and DD programs. Hammer Care Coordination provides services contracted with Medica for people ages 18-64 enrolled in a Special Needs Basic Care health plan. Primary Duties and Responsibilities

1. Administrative Support

· Mailing, compiling documents, and requesting documentation from outside agencies

· Maintain accurate client records, including paper documents, scanning, and managing electronic storage

· Maintain electronic client database

· Maintain detailed tracking systems

· Interact professionally with external business partners

· Complete tasks by assigned deadlines

· Remain up to date with programmatic changes in the department

· Organize multiple responsibilities at once, requiring time management and organizational skills

2. Documentation

· Document case notes in electronic record system

· Maintain case notes for all work throughout the day

o Must include a detailed, objective narrative of all tasks completed

o Include precise time of each completed task to allow for necessary billing of work

· Ensure integrity in all documentation completed

Requirements

 

Essential Knowledge and Qualifications

· High school diploma

· 1-year experience in administrative support role preferred

· Excellent written and verbal communication skills, including fluency in English

· General computer competency, including typing, email, and managing electronic documents

· Proficiency with Microsoft Office (particularly Excel, Outlook, and Word)

· Strong organizational skills

· Strong attention to detail

· Flexibility to adapt to frequent process changes

· Self-motivation to complete tasks independently

· Willingness to ask questions

· Ability to think critically and problem-solve independently

· Ability to maintain tracking systems for multiple tasks to meet varying deadlines

· Ability to maintain confidentiality

· Ability to work effectively as a team member
