Description

At Konza Prairie Community Health Center, we are seeking a compassionate and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our integrated care team. This is a full-time opportunity to provide patient-centered care in a collaborative, mission-driven environment/


As a vital member of our care team in a Federally Qualified Health Center (FQHC), you will support the delivery of outpatient primary care alongside providers, behavioral health professionals, and fellow clinical staff. Our model emphasizes whole-person, evidence-based care—giving you the time and support to focus on quality, build meaningful patient relationships, and make a lasting impact in the communities we serve.


What You’ll Do

  • Perform patient assessments, triage, and document medical histories
  • Collaborate with providers and care team members to deliver high-quality primary care
  • Accurately maintain patient records in the electronic medical record (EMR)
  • Administer medications, immunizations, and treatments to patients of all ages
  • Assist with procedures and coordinate care, including follow-up and referrals
  • Provide patient education and triage phone calls to support ongoing care

Requirements
  • Active Kansas RN or LPN license
  • Current BLS certification
  • Strong working knowledge of EMR
  • 1+ year of experience in primary care, ambulatory care, or community health preferred
  • Experience in a FQHC preferred
  • Bilingual (Spanish) preferred
