Position Summary


Under the direction of the Director of Quality Outcomes and Utilization, the Referral Coordinator serves as a key liaison between Bond Community Health Center, patients, and external healthcare providers. The primary responsibility of this position is to coordinate patient referrals and ensure that referral appointments are scheduled timely, required authorizations are obtained, and reports and other documentation are received and accurately tracked.


The Referral Coordinator plays an important role in supporting continuity of care by maintaining effective communication with patients, primary care providers, managed care organizations, and external specialists. This position also provides administrative and operational support to the Retina Vue Examination Program.

Requirements

Essential Duties and Responsibilities

  • Schedule patient referral appointments with external  healthcare providers within seven (7) to fourteen (14) business days of      the provider's request, when applicable.
  • Accurately document referral activity, appointment information, and follow-up actions in the appropriate systems.
  • Obtain and process managed care authorizations for referral services as required.
  • Communicate with patients regarding referral appointments, scheduling information, and other referral-related needs.
  • Follow up with external providers to confirm receipt of patient reports following referral appointments.
  • Maintain accurate referral logs to track scheduled appointments and match received reports to completed referrals.
  • Monitor outstanding referral reports and identify  referrals requiring additional follow-up.
  • For reports not received within two (2) weeks of the scheduled appointment, contact the external provider to confirm whether      the patient attended the appointment and request the missing report or documentation.
  • Communicate with the patient's primary care provider when a referred appointment was not attended or when additional follow-up is needed.
  • Assist with identifying and resolving referral scheduling or documentation issues that may impact continuity of patient care.
  • Prepare and submit weekly referral reports, including  appointment scheduling, referral status, outstanding reports, and other      applicable tracking metrics.
  • Maintain accurate, organized, and confidential referral records.
  • Provide administrative and coordination support for the Retina Vue Examination Program.
  • Collaborate with clinical and administrative staff to facilitate timely completion of referral-related activities.
  • Demonstrate excellent customer service when interacting with patients, providers, staff, and external healthcare      organizations.
  • Protect patient confidentiality and comply with organizational policies, procedures, and applicable privacy requirements.
  • Participate in staff meetings, training, and other departmental activities as required.
  • Perform other related duties as assigned.

Minimum Qualifications


Education & Experience

  • High School Diploma or equivalent required.
  • Prior experience working in a healthcare or medical setting required.
  • Working knowledge of medical terminology required.
  • Previous experience coordinating referrals, scheduling medical appointments, or working with insurance/managed care      organizations preferred.
  • Experience working with electronic health records,  referral management systems, or medical scheduling software preferred.

Knowledge, Skills & Abilities

  • Proficiency with computers, electronic health records, scheduling systems, and relevant software applications.
  • Strong knowledge of medical terminology and healthcare office practices.
  • Excellent customer service and interpersonal communication skills.
  • Strong verbal and written communication skills.
  • Excellent organizational and time-management skills.
  • Strong attention to detail and accuracy.
  • Ability to maintain referral tracking logs and meet established deadlines.
  • Ability to communicate effectively with patients, healthcare providers, insurance/managed care organizations, and internal      staff.
  • Ability to follow established procedures and independently manage routine referral follow-up activities.
  • Ability to prioritize multiple assignments while maintaining accuracy and timely completion.
  • Ability to handle confidential patient information with professionalism and discretion.
  • Demonstrated dependability, professionalism, and commitment to patient-centered service.

Work Environment

This position works in a professional healthcare environment and requires frequent communication with patients, healthcare providers, managed care organizations, and internal staff. The Referral Coordinator must be able to manage multiple referrals and follow-up activities while maintaining accuracy, confidentiality, and timely service.
