This is a full time, direct care position. The primary responsibilities of the Extender are to:
- Perform general outreach, engagement, and follow-up with members
- Spanish language proficiency preferred
- Coordinate services/appointments
- Engage clients in health promotion activities
- Share information with Care Manager on the member's circumstances
- Make and track referrals
- Participate in case conferences
- Support the care manager in assessing and addressing unmet health-related resource needs.
Must be a be at least 18 years of age, have a high school diploma (or equivalent)
1-2 years paid experience preferred within behavioral/mental health field.
Spanish speaking/bilingual preferred.
