Description

Bridges of Kentucky, Regional Office is seeking independent contractors to become Family Host Home Providers.

A Family Host Home is a residential program model providing individuals with intellectual and developmental disabilities a stable home environment. The program is designed to allow individuals to live with a family or individual(s) in their desired environment. Family Host Home Providers provide supervision and training to the individual(s) supported in their private home. Individuals’ supported in a Family Host Home setting have various needs, such as assistance with mobility, personal care, etc.

Family Host Home Providers may work with up to two individuals with disabilities in their home, depending on the number of bedrooms/bathrooms and the amount of shared space that is available. Individuals can choose their roommates, and a 30-day trial period is required to ensure the Family Host Home is an appropriate placement for the individual(s).

Bridges of Kentucky supports individuals with intellectual and developmental disabilities who have a wide range of abilities and areas in which they need support. It is our goal to find the perfect home for our individuals; a home in which they can prosper and grow and where they are welcomed as a member of the family.


WHERE ARE FAMILY HOMES LOCATED?

Bridges of Kentucky has family homes throughout Kentucky. Each family home offers different areas of specialty, has different layouts and works on meeting individual needs.


WHAT DOES A FAMILY HOME PROVIDER DO?

The responsibilities of a Family Home Provider vary depending on the level of support required by the individual. Examples of support include assistance with bathing, transportation, scheduling and attending appointments, household tasks, behavior supports and other activities of daily living. All Family Home Providers are trained by Bridges of Kentucky to ensure they are competent in completing the daily record keeping required by the Commonwealth of Kentucky.


Bridges of Kentucky’s family homes are all monitored closely and we ensure that each consumer is receiving the highest level of care.

  • Room and board based on individual social security amounts paid out with no percentage taken out from Bridges of Kentucky





Requirements

Some basic requirements to become a Family Host Home Provider/Independent Contractor include but are not limited to:

  • Be at least 18 years old
  • High school diploma or equivalent.
  • Have a house or apartment, rented or owned. The home must provide the individual(s) with a private bedroom and a bathroom in accordance with state regulations.
  • Successfully clear a background check, including criminal history, good driving record and reference checks of former employers and personal references. Applicants who do not have a valid driver's license may still be considered if another family member does have a valid license and acceptable driving record and is available to transport the individual whenever needed.
  • Carry full insurance coverage on your automobile, which meets the minimum state liability requirements and full home/renter's insurance which is adequate to cover the possessions of the individual(s) living with the contractor.
  • Be established and available in order to sign a one-year (or other elected time-frame), renewable contract with BOC.
  • Own and be able to use a computer with internet service and have all required documentation equipment and supplies needed to care for the individual(s) in the home
  • Must possess the ability to deal tactfully with clients, family members, visitors, government agencies/personnel, and the general public.
