JOB DESCRIPTION:

Advance Nursing Services recruits on behalf of Holliday’s Helping Hands and Serenity Recuperative Care which are organizations that care for homeless families and individuals - We support the cause and fight to end homelessness!  Our jobs focus on providing interim housing, nourishing meals, and safe locations to empower our clients to find their “Journey to Home.”

Core Responsibilities

• Provide medication management to individuals including but not limited to medication observation, treatment and monitoring of side effects, monitoring and supportive services to increase compliance
• Inform Psychiatrist, Nurse Practitioner (NP), Primary Care Physician (PCP), Case Manager, and mental health provider, as appropriate, of medication concerns/issues
• Provide case management by coordinating and implementing mental health and recovery support services with individual members
• Document all services provided in clinically appropriate progress notes with timeliness and accuracy
• Coordinate client services and appointments by working closely with the NP and providing follow-up consultation to clients
• Prepare and ensure proper completion of the Nursing/Medical Initial Intake documentation including nursing notes, Medication Observation Record (MOR)
• Maintain medication logs and medication storage in compliance with safety standards and industry standards of care
• Other duties as assigned
