Location: North Central Massachusetts

Schedule: Full-Time (35 Hours Per Week)

Compensation: $34-$37 per hour

Aging Services of North Central Massachusetts (ASNCM) is seeking a Community Assessment Nurse (RN) to join our mission-driven team.

Rather than providing direct patient care, administering medications, or performing treatments, you will use your nursing expertise to assess consumer needs, identify risks, recommend services, and help coordinate supports that promote independence, safety, and quality of life.

What You'll Do

As a Community Assessment Nurse, every day is different. You may:

? Visit older adults in their homes to assess their health, safety, and support needs.

? Build meaningful relationships with consumers and family caregivers.

? Complete comprehensive nursing assessments and identify areas of concern.

? Recommend services and supports that help consumers remain safely in their homes.

? Collaborate with care managers, physicians, hospitals, rehabilitation facilities, and community providers.

? Educate consumers and caregivers about community resources and service options.

? Assist with transitions from hospitals, rehabilitation facilities, and nursing facilities back into the community.

? Advocate for consumers and help them navigate healthcare and long-term care systems.

? Monitor changing needs and recommend adjustments to care plans when appropriate.

? Maintain accurate documentation and complete required assessments in accordance with program guidelines.

What We're Looking For

Required Qualifications

• Active Massachusetts Registered Nurse (RN) license in good standing.

• Strong assessment and critical thinking skills.

• Excellent communication and interpersonal skills.

• Ability to work independently and manage a flexible schedule.

• Strong organizational and documentation skills.

• Valid driver's license.

• Access to a legally insured and registered motor vehicle.

• Ability to travel throughout North Central Massachusetts.

Preferred Qualifications

• Experience in community health, home health, geriatrics, care management, rehabilitation, or long-term services and supports.

• Experience working with older adults and family caregivers.

• Strong relationship-building and consumer engagement skills.

• Interest in community-based nursing and supporting independent living.
