DISTINGUISHING FEATURES OF WORK:

- Directly relates to the Site Manager for administration and program issues

- Works with the QMHP/QIDP for clinical issues

- Is responsible for the consumers while they are in the home to see that they are safe and following their treatment plan

- Also responsible for overseeing the homes


ILLUSTRATIVE DESCRIPTION OF DUTIES:

- Provide direct service care as required

- Assist consumers with implementing their Individual Treatment Plan in their homes, community and other environments

- Help consumers with crisis situations involving supervisors and clinical staff as needed/required

- Assist consumers with the administration and training of medications taking direct orders from certified medical personnel

- Make sure that the home is a safe and clean environment, i.e. – assisting in emergency drills, checking safety equipment

- Assist consumers in purchasing and preparing food and other items as needed for them in their home

- Transport consumers in agency vehicles to appointments and activities in the community as outlined in their Treatment Plan

- Help consumers to participate in the design of the array of community support services tailored to their needs

- Maintain sensitivity to the service population’s cultural and socioeconomic characteristics.

- Be responsible for staying updated on changes in consumers’ treatment plans, medications and overall care


ADMINISTRATIVE DUTIES:

- Follow policies and procedures of the site, CSN and Human Support Services

- Follow and implement all medication procedures as assigned 

- Participate in the planning of consumers’ Individual Treatment Plan

- Do related progress notes  per the goals assigned to you for billing purposes and provide all the services indicated in the treatment plan and all other documentation necessary in a timely manner

- Provide necessary information regarding consumer care to site manager and/or appropriate staff in a timely manner


TRAINING AND JOB REQUIREMENTS:

- Meet with Site Manger monthly at scheduled site meetings to assist with consumer monitoring and planning

- Attend full CSN trainings quarterly to stay updated on consumer care and/or changes affecting CSN and Human Support Services

- Successfully complete all required certifications of CSN, HSS and the State of Illinois that relate to your position as scheduled

- Attend trainings outside required monthly trainings that relate to your position as mutually agreed upon


OTHER: 

- Be available to fill shifts as needed

- Successfully complete orientation program

- Able to provide basic living skills to adults

- Work within agency structure and accept supervision

- Work constructively using team approach with peers

- Work with providers in the community

- Must feel confident to report abuse and neglect of consumers/staff to supervisor

- Must be flexible and open-minded

- Do other duties as assigned and/or as mutually agreed upon

Requirements

- Must be 21 years of age

- Must have valid driver’s license

- Have adequate transportation to be able to move from site to site

- Ability to drive a regular size and larger size van or willing to be trained

- Drive in large cities

- Present original Social Security card

- Present High School diploma or GED

- Able to lift 50 pounds and walk flights of stairs

- Have normal or corrective eyewear and hearing devices

- Have clarity of speech to be understood

- Good hearing

- Constitution to handle body excrements

- Must be emotionally stable enough to react appropriately to a consumer in crisis
