Description
Provides individual services to adult, substance-using clients by following evidence-based practices; assists clients in linking to recovery support services, health care and other needed services; and develops individual Service Plans that help clients enhance self-efficacy, maintain engagement in medical care, and reduce HIV risk and transmission behaviors.
ESSENTIAL FUNCTIONS:
- Carries caseload of patients living with HIV at Haymarket Center
- Assist patient in receiving necessary support services such as medical care, housing, and public benefits
- Foster participation in recovery support services
- Develop and update initial patient service plan with patient input
- Maintain advocate position for patient through treatment episode and throughout housing in recovery home setting
- Complete all necessary documentation and reports in Electronic Care Record in a timely manner
- Facilitate didactic and therapeutic groups when required
- Develop appropriate community resources such as case management, mental health services, substance abuse treatment, food, housing, and public benefits among others
- Provide education and support for engagement in medical care, medication adherence, and HIV risk reduction techniques.
- Support client through navigation of health care
- Collaborate with other service providers to create network of providers to ensure client linkage to services and recruit participants for Haymarket Center programs and services
- Attend programmatic meetings, staffing, trainings, and supervisions
- Follow agency regulations regarding vehicle use
- Other duties as required for effective team work or as assigned by the Manager
Requirements
High School Diploma with 2 years of Case Management Experience
A Bachelors degree in Human Services (Preferred)
EXPERIENCE AND SKILLS:
- Ability to work with diverse populations, cultural backgrounds, and sexual orientations
- Knowledge about HIV/AIDS
- Previous case management experience beneficial
- Certified as Case Manager by AIDS Foundation of Chicago preferred
- Bilingual (English/Spanish) preferred
- Proficiency in reading and writing
- Able to work effectively on a team
- General computer literacy is mandatory
- Previous experience working with an Electronic Health Record (ECR) highly preferred
- Maintain a valid Illinois Driver's License and eligibility for agency insurance coverage
Salary Description
$42,640.00 - $64,140.00
