LICENSED PRACTICAL NURSE
JOB SUMMARY: Responsible for the health and welfare of the people we supported by Sertoma, especially ensuring quality medical services, and coordinating with doctors, pharmacies, hospitals and Sertoma residences for continuation of care. Monday through Friday; 8 am - 4:30 pm May occasionally fill in at the medical residential house.

REPORTS TO: Agency Registered Nurse
WORKS WITH:
Senior Leadership, Residential Services, Day Services, Case Managers, ECF, Human Resources, Event Management Coordinator, supported individuals, direct care staff
SUPERVISES: None
EXTERNAL RELATIONSHIPS:
Independent Support Coordinators, DDA, MCOs, pharmacy, physicians, family members, conservators, and any other contacts necessary to the coordination of medical services
QUALIFICATIONS:
EDUCATION:
Must be a currently Licensed Practical Nurse in the State of Tennessee
WORK EXPERIENCE:
Experience with ID/DD population preferred
LICENSE OR REGISTRATION REQUIREMENTS:
Valid TN driver's license and a good driving record and a current LPN license in good standing
PHYSICAL REQUIREMENTS:
Sight, hearing, moderate physical strength and good mobility needed to assist individuals, lift wheelchairs, etc.
RESPONSIBILITIES:
1. Assist RN in the coordination and provision of medical services for the individuals
supported by Sertoma.
2. Provide medical support to the individuals which may include triage prior to a medical appointment or to determine the need for a medical appointment and communicating findings to MD office.
3. Ensure medical orders are implemented by support staff through periodic unannounced
monitoring visits to each site.
4. Maintain ongoing communication with staff as appropriate, keeping staff informed of all
pertinent medical information.
5. Assist RN in ordering medication, checking in medication when delivered and distributing medication to the appropriate staff.
6. Assist RN in reviewing monthly Medication Administration Records (MARs) prior to
distribution to the residential sites for accuracy.
7. Monitor documentation follow up from medical providers (labs, x-ray/scan results, office notes, etc.) and enter into TIMAS
8. Assist in counseling individuals as needed.
9. Monitor blood pressure or blood glucose readings of designated individuals as needed per MD orders and document the readings in TIMAS. Documentation will be provided to the physician during scheduled appointments.
10. Assist in maintaining a medication history on each individual supported.
11. Monitor weights as recommended by the individual’s physician, documenting the results in TIMAS.
12. Provide and monitor sick slips as appropriate, for individuals who take medication for constipation, sleeping difficulties, menses cycle, incontinence, PRN medication doses, or general illness, etc.
13. Serve in the absence of the RN in coordinating the activities of the Clinic.
14. Participate in rotating weekend “on call” responsibilities with the RN.
15. Adhere to an acceptable level of professionalism to ensure the maintenance of
confidentiality and discretion necessary in the maintenance medical records.
16. As a representative of Sertoma, maintain professionalism in all interactions with
outside entities.
17. Contribute to the "effort" of the Center itself by continually being alert to new concepts which could improve or enhance either your job individually or the collective functions of the Center.
18. Observe all Center routine regarding work schedules, presence during working hours which conform to those of the Center, designated periods of break time, lunch hour, etc.
19. Perform all work assignments and activities necessary to the professional operations of the LPN position in an acceptable manner and require minimal supervision from the RN.
20. Perform any other duties as assigned by the RN or member of senior leadership.
