HSS is a private, non-profit community resource agency founded in 1973, providing confidential COA-accredited services. We are seeking a compassionate and organized Mental Health Case Manager to support individuals experiencing mental health challenges. This role provides client-centered assessment, coordination of care, advocacy, and resource linkage to help clients achieve stability, independence, and improved well-being. The Case Manager works closely with clients, families, clinical staff, and community partners to ensure seamless, high-quality care.

DISTINGUISHING FEATURES OF WORK:


Directly reports to the Clinical Director for administrative and program issues. Is responsible for case management, Community Support services, emergency services, community education, supervision of practicum students, and documentation of clinical services, program and self-development.


ILLUSTRATIVE EXAMPLES OF POSITION DUTIES:

  • Meet with the Clinical Director/Assistant Clinical Director monthly and with other program staff for administrative program planning meetings. Participate in agency staff meetings, medical staffing's, supervision, clinical staffing's, annual evaluations, and other appropriate agency meetings.
  • Provide Community Support Individual and Community Support Group services when included as goals in the treatment plans of assigned clients. Services are to be provided in client homes as well as in other appropriate community settings and in agency facilities.
  • Provide support for the clinic's 24-hour crisis assistance service by handling calls when contacted. 
  • Provide case management including assessment, planning, coordination and advocacy services for clients who need multiple services and require assistance in gaining access to and using medical, social, vocational, educational, housing and other community services. This includes identifying and investigating available resources, explaining to clients and making linkages. Special attention required to assist clients in maintaining participation in Medicaid, Medicare, SSI disability services.
  • Maintain sensitivity to the service population’s cultural and socioeconomic characteristics. 
  • Participate as needed in peer review and program evaluation programs.
  • Complete all necessary clinical documentation such as assessments, treatment plans, and progress notes in accordance with agency standards regarding timeliness, content and format. 
  • Meet agency standards for billable services.
  • Provide community education by making presentations at schools, social clubs, newspaper articles, etc.
  • Continue to acquire new clinical knowledge and abilities in order to improve outcomes of clinical services provided.
  • Other duties as are necessary or related to the successful completion of other stated position duties, or as mutually agreed. 
Requirements

* Minimum of a Bachelor's degree in a Human Services field (Psychology, Social Work, Counseling, etc.)

* One year relevant job experience in case management preferred.

* Must be willing to work in a multidisciplinary team environment.

* Must be a self starter and able to manage time

* Must have a valid driver’s license and be insurable for auto coverage.
