Our waterfront community consists of 300 Independent Living Residential Units, a 33-bed (Medicare/private) Skilled Nursing Healthcare Unit, a 40-bed Assisted Living Unit, and 34-bed Memory Support Unit. 

 

We are seeking a positive, highly motivated, skilled full-time LPN Day Supervisor (8 & 12-hour shifts available) with strong leadership and clinical skills to join our teams in Assisted Living and Memory Support Units, under the direction of the AL/MS Manager and/or Health Services Administrator. The Supervisor will be responsible to provide appropriate supervision to other staff as well as direct care, support, positive direction, and assistance to residents in accordance with current federal, state, and local standards, guidelines, and regulations, as well as each resident's individualized support plan and personal outcomes. The LPN Supervisor enforces policies and procedures, assists with staffing plans, reports and budgets.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Supervise activities of the Assisted Living and Memory Support departments in compliance with all applicable rules, regulations, and guidelines, under the guidance of the MS Manager and Health Services Administrator, or resident's physician as directed. 
  • Make reports and recommendations to the Administrator as necessary or required concerning the operation of the departments. 
  • Accountable for making recommendations on hiring, terminations, disciplinary action, training and promotions. Create and assign work schedules and ensure appropriate staffing levels. Orient and train new employees, assisting them with onboarding to the department and the facility.
  • Oversee development of and compliance with resident care plans, providing direct care to residents as needed. Observe residents for change in condition and report any changes to Administrator and resident’s physician, as directed. Document all pertinent/relevant observations, communication with the resident’s physician & responsible party, clinical procedures and daily assistance/care by the end of each shift. Handle routine problems and concerns of residents, escalating serious issues to Administrator for review/action plan.
  • Ensure all protocols and procedures are followed. Monitor ongoing operations and participate in assessing compliance with established QA standards. Identify staff educational needs and coordinate training with Administrator.
  • Provide excellent direct nursing care services to residents, perform assessments as required by regulations and facility policy, following individual resident care plans (ADL Assignment Worksheets). Update, complete, and maintain ISP for each resident. Participate in evaluation of new residents and level of care transitions. Perform assessments and help admit and discharge residents, under the direction of the MS Manager, the Health Services Administrator, and scope of practice as required by regulations and facility policy. 
  • Administer medications, injections, and other treatments as authorized by current licensure by the VA State Board of Nursing, assessing safety of all medication, including narcotics, with inter-shift count. Check and record vital signs, weights, and blood sugars as assigned. Observe residents for change in condition and reports any changes immediately to MS Manager and/or Health Services Administrator, as well as resident’s physician as directed. 
  • Ensure documentation of all pertinent/relevant observations, communication with resident’s physician & responsible party, clinical procedures, and daily assistance/care is completed by the end of each shift. 
  • Assist residents with activities of daily living as needed, including bathing/showering according to schedule, oral care, toileting, dressing, etc.
  • Encourage residents to participate in recreational activities and assist transporting residents to and from activities.
  • Respond to urgent calls as needed
  • Develop methods for coordination of the departments’ services with other resident services, to ensure the continuity of the residents' total regimen of care. Attend scheduled interdisciplinary resident care meetings.
  • Assist with departmental planning and budgets. Arrange for appropriate supplies and equipment, maintaining compliance with departmental budget.
  • Communicate effectively and tactfully with residents, family members, physicians, management, staff, and the public in a timely manner.
  • Help with planning, organizing, and functioning of various resident committees, communication, and Community newsletter as needed.
  • Attend in-service classes, required training, and staff meetings. Assist in developing/updating written policies and procedures and job descriptions.
Requirements

REQUIREMENTS:

  • Minimum of High school diploma/GED.
  • Current state license as an LPN in the Commonwealth of Virginia.
  • At least three (3) years of related work experience; Assisted Living and/or Long-Term Care experience required.
  • At least one (1) year of nursing supervisory experience required. 
  • Current COVID Vaccine is required to support the health and safety of our residents and staff.
  • Must have CPR and First Aid Trained with a current, valid certificate/card.
  • ISP/UAI certification preferred
  • Experience with Electronic Medical Record system preferred
  • Must be able to exert at least 50 lbs. of energy in order to assist residents in and out of wheelchairs, beds, etc., as well as be able to sit, stand, walk, climb stairs, reach, turn, bend, stoop, crouch and kneel, balance and maintain body equilibrium to prevent falls, reach over the head, push/pull, and grasp with both hands. 
  • Must demonstrate excellent communication skills, including verbal and written.
  • Must be able to successfully follow verbal and written instructions in a timely manner.
  • Must be able to communicate tactfully and effectively with personnel, residents, family members, visitors, and the general public.
