Position Summary
The Physical Therapist Assistant (PTA) works under the direction and supervision of a licensed Physical Therapist to deliver individualized therapy services that help residents improve mobility, restore function, reduce pain, and maximize independence. This role collaborates closely with residents, families, nursing staff, and the interdisciplinary team to support exceptional clinical outcomes while maintaining compliance with professional practice standards and regulatory requirements.
Key Responsibilities
- Provide physical therapy treatments and therapeutic interventions as directed by the supervising Physical Therapist.
- Assist residents in improving strength, mobility, balance, endurance, and functional independence.
- Observe resident response to treatment and communicate progress, concerns, or changes in condition to the supervising Physical Therapist.
- Assist with implementing individualized treatment plans and resident education programs.
- Encourage residents and families throughout the rehabilitation process while promoting dignity, independence, and quality of life.
- Maintain accurate, timely, and compliant clinical documentation in accordance with facility policies and professional standards.
- Collaborate with physicians, nursing staff, rehabilitation professionals, and the interdisciplinary care team to ensure coordinated resident care.
- Participate in quality improvement initiatives, infection prevention programs, departmental meetings, and continuing education activities.
- Maintain therapy equipment and treatment areas to ensure a clean, safe, and organized environment.
- Adhere to all facility policies, infection prevention protocols, safety standards, and regulatory requirements.

Required Qualifications
- Graduate of an accredited Physical Therapist Assistant (PTA) program.
- Current Physical Therapist Assistant license in the State of Kentucky.
- Current CPR certification (or ability to obtain within required timeframe).
- Ability to provide physical therapy interventions under the supervision of a licensed Physical Therapist.
- Strong communication, interpersonal, and organizational skills.
- Commitment to compassionate, resident-centered care.
- Ability to meet the physical requirements of the position, including standing, walking, lifting, transferring residents, and assisting with therapeutic activities.
Preferred Experience
- Previous experience in skilled nursing, long-term care, inpatient rehabilitation, outpatient therapy, or hospital settings.
- Experience treating geriatric and post-acute rehabilitation populations.
- Knowledge of Medicare Part A, Part B, and rehabilitation documentation requirements.
- Experience collaborating within an interdisciplinary healthcare team.
- Strong clinical reasoning, time management, and documentation skills.
