Description
Weekly Hours: Full Time - 40 hours per week
Location: Must be willing to travel between all Brio locations depending on schedule needs (Harrison Bridge Rd, Simpsonville and Bella Grove Rd, Greenville)
Position Overview: Works independently with the Diabetic population to perform Diabetic Care Visits, identify gaps in care, safety concerns, and social needs. Prepares charts for scheduled visits.
Responsibilities:
- Takes ownership of ensuring the “Brio Way” is experienced by Brio patients, families, and internal team members. Promotes an authentic, transparent work environment within the clinical areas.
- Helps carry out team short term and long-term goals for Diabetic Care Team (DCT) and ensure efficiencies with processes.
- Prepares charts for scheduled Diabetic Care Visits with patients and ensures patient is up to date on all routine screenings and immunizations.
- Will have face to face time, approximately 45 minutes up to 1 hour, with each patient to ensure patient’s chart is up to date, perform vitals, administer vaccinations, and perform routine screenings such as ABI, Diabetic Eye Exam, and EKG.
- Provide communication with providers regarding patient needs identified in scheduled Diabetic Care Visit, such as gaps in care, safety concerns and social needs, for their upcoming follow-up visit.
- Help to identify patients eligible for DCT appointments and schedule/alert provider when needed.
Requirements
Required Education:
- CMA or RMA required
- Comfortable with computer skills and excellent typing
- Must be comfortable practicing candor and giving feedback to team members when appropriate.
- Medical office experience preferred but not required
- Excellent interpersonal and communication (oral and written) skills
Work environment and Physical Demands: Requires sitting, standing, and walking associated with normal physician office environment. Person may be exposed to fumes, airborne particles, infectious diseases, blood/bodily fluids, and disease-bearing specimens.
