Description

SGA provides a wide range of social services in some of Chicago's most challenged and consistently underserved communities, creating long-term and community-wide change. SGA’s mission is to help children, families, and communities facing great challenges to realize their potential. SGA uses a proven and innovative method called the Cycle of Opportunity. This system is a continuum of comprehensive services that helps break the cycle of adversity and provides families with enough opportunity to change their lives and their communities.



MAJOR DUTIES AND RESPONSIBILITIES

• Respond to incidents of community violence, their families, and those at highest risk for involvement 

• Advocacy: Working with local government and law enforcement to advocate for policies and practices that support violence reduction and community safety

• Create goal-planning processes, short-term and long-term, and service plans to promote family well-being and find new strategies to address new needs.

• Collect resources on recreational activities, housing, food, clothing, school programs, vocational  opportunities or services, providers to teach life skills, and relevant mental health services

• Assists clients in developing natural resources and contacting social support networks.

• Provide case management services through one-on-one meetings with families to determine and meet their needs.

• Assist families in applying for public benefits and services, interpretations may be needed.

• Verbal or electronic translations of brochures, forms, and application instructions, as required, to access services

• Review to ensure that all eligibility requirements are met, including uploading required documentation

• Establish MOUs with community partners to create a referral/linkage system for clients enrolled in the program.

• Community Outreach: Building relationships with community members, local organizations, and stakeholders to foster trust and collaboration



Requirements

  

  • High School, GED, or lived experiences 
  • At least two years’ experience working with youth and families in designated communities
  • Must be committed to working with young people and their families who are affected by violence and have a flexible schedule including weekends and holidays.
  • Must have transportation, valid driver's license and insurance




Salary Description
$23.00 Hour
