Position: LVN - Licensed Vocational Nurse

Job Type: Full-time and Part-Time Available 

Available Shifts: Mon-Fri 7am-3pm (90023); Mon-Fri 3pm-11pm (90033); Sat - Sun 7am-3pm (90033 or 90043); Sat-Sun 3pm-11pm (90033) (flexible schedules if needed)

Starting Salary Range: $30 to $34 an Hour

JOB SUMMARY


The LVN will take a leadership role in medication and health issues in the SRC Care Program and will ensure the quality and adequacy of nursing services provided to members. 


ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include:

  • Provide medication management to individuals including but not limited to medication observation, treatment and monitoring of side effects, monitoring and supportive services to increase compliance
  • Inform Psychiatrist, Nurse Practitioner (NP), Primary Care Physician (PCP), Case Manager, and mental health provider, as appropriate, of medication concerns/issues
  • Provide case management by coordinating and implementing mental health and recovery support services with individual members
  • Document all services provided in clinically appropriate progress notes with timeliness and accuracy
  • Coordinate client services and appointments by working closely with the NP and providing follow-up consultation to clients
  • Prepare and ensure proper completion of the Nursing/Medical Initial Intake documentation including nursing notes, Medication Observation Record (MOR)
  • Maintain medication logs and medication storage in compliance with safety standards and industry standards of care
  • Other duties as assigned
Requirements

QUALIFICATIONS
Education & Experience

  • Current, clear California license as a Licensed Vocational Nurse
  • Experience working with the homeless, substance abuse, mental health/ with dual diagnosis population preferred
  • Bilingual Spanish/English is preferred but not required
  • Comfortable working at a traditional residential housing facility 

Computer Skills

  • To perform this job successfully, an individual should have strong knowledge of MS Outlook, MS Excel, MS Word, and Internet software.
