Job Summary:
Essential Functions:
• Set up appointments for CASAC if necessary
• Record assessment information into access database
• Schedule CASAC reviews
• Obtain all attendance sheets for open D/A cases
• Monitor clients progress in treatment
• Schedule reviews with CASAC as determined
• Maintain files on all clients with current paperwork, releases, attendance and discharges
• Complete monthly statistic logs
• Complete semi – annual RBA report
• Work with employment and under care to ensure clients remain in compliance
• Send out appointment letters and other communication to clients
• Completing employment assessments and restoration to self-sufficiency plans
• Assisting individuals with locating the appropriate treatment based on CASAC recommendations and the district’s final determination.
• Work with drug court, employment and under care to ensure clients remain in compliance
• Participate in staff meetings, training sessions, and professional development activities as required.
• Other duties as assigned.
Qualifications:
• Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).
• Proven experience in community service coordination or related social services work.
• Strong understanding of substance abuse disorders, mental health issues, and trauma-informed care principles.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a team in a fast-paced environment.
• Current and valid New York State driver’s license
