SUMMARY
The LVN will take a leadership role in medication and health issues in the Recuperative/HHH Care Program and will ensure the quality and adequacy of nursing services provided to members.
ESSENTIAL DUTIES & RESPONSIBILITIES:
1. Provide Case Management; coordinate and implement mental health and recovery support services with individual members (caseload max 10)
2. Document all services provided in clinically-appropriate progress notes with timelines and accuracy
3. Provide medication management to individuals including but not limited to medication administration, treatment and monitoring of side effects, monitoring and supportive services to increase compliance, etc.; inform psychiatrist, case manager, and mental health provider, as appropriate, of medication concerns/issues
4. Coordinate SRC/HHH psychiatric services and appointments by working closely with psychiatrist and providing follow-up consultation to patients
5. Train and provide indirect oversight of medication to client staff
6. Prepare and ensure proper completion of the Nursing/Psychiatry Initial Intake documentation, including:
a. Initial Medication Support Service Form
b. Outpatient Medication Review Form
c. Completion of Psychiatrist MED Goal on CCCP with doctor signature
d. Completion of nursing goal with signature (if applicable)
e. Change of diagnosis (if applicable)
f. Case Management notes
g. Lab requisition requests and other associated documents, as requested
7. Maintain medication logs and medication storage in compliance with safety standards and industry standards of care
8. Provide medication consultation to staff and patients as requested
9. Develop and conduct Health/Wellness and Medication Management Groups
10. Develop and implement nursing care plans and maintain medical records
11. Coordinate medication support services with staff and provide direct linkage and/or support to Case Managers to ensure that members attend medical appointments for psychiatric medication monitoring blood work, and obtaining psychiatric medications when prescribed
12. Maintain confidentially of patients files per HIPAA and all applicable guidelines
13. Utilize proper lifting and body mechanics to prevent personal injury
14. Accurately input data into Data Base and logs
WORK ENVIRONMENT
1. Combination of field and office environment
2. On occasion, walk or drive to different local sites
3. May necessitate working in busy and loud environments
4. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds
5. May be exposed to outside elements like cold, heat, dust, noise and odor
6. May need to bend, stoop, twist, and sit throughout the day

QUALIFICATIONS
1. Proof of graduation from an approved nursing program with an associate’s degree or equivalent
2. Current, California license as a Registered Nurse or Licensed Vocational Nurse with a clear license
3. Minimum one year psychiatric nursing experience working with clients experiencing chronic homelessness and substance abuse
4. Able to physically lift, move, and position patients as needed.
5. Able to manage multiple tasks, projects, and priorities effectively while working independently
6. Detail oriented with excellent time management, organizational, written, verbal, and computer skills
7. Able to demonstrate acceptance toward patients and a high degree of cultural sensitivity to their needs
8. Able to develop patient care programs within the clinical policies and program objectives and relating them to the funding requirements
9. Experience completing medication logs to accepted industry standards
10. Able to produce data for reports at any given time, ongoing basis, and at “last minute” request
11. Minimum 30 WPM typing speed preferred
12. Bilingual in English/Spanish preferred
