As a Licensed Practical Nurse at LIFE Pittsburgh, your work has real weight. You’ll serve older adults across Allegheny County—not in a hospital, but in their homes, communities, and at our Day Health Centers. You’ll be the face they trust and the steady hand they rely on.


Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community.

What You’ll Do

  • Provide hands-on personal and clinical care (wound care, medication administration, respiratory support)
  • Monitor vital signs and report changes to the RN or IDT
  • Assist with medication box preparation and physician order follow-up
  • Maintain accurate documentation and communication
  • Help Participants remain safe and supported in their homes

Your day may start at one of our welcoming Day Health Centers in Greentree, Northshore, Allegheny Center, or McKees Rocks, but your impact reaches far beyond. Our nurses sometimes travel throughout Allegheny County to deliver care where it matters most—in the community, in homes, and in the hearts of those we serve.


You’ll serve older adults throughout Allegheny County, providing care at our Day Health Centers and in Participants’ homes, community spaces, or local healthcare facilities—wherever support is needed most.

Requirements

 What You’ll Need

  • Active PA LPN license
  • Minimum one year of experience  with the elderly and or frail.
  • CPR certification
  • Valid driver’s license, reliable  vehicle, and auto insurance
