Assisted Outpatient Treatment Case Manager (AOT)

Title: Assisted Outpatient Treatment (AOT) Care Manager- Position located in Middletown-in person

Reports to: Assisted Outpatient Treatment (AOT) Supervisor


Job Objectives:


A. Provide AOT Health Home Plus Care Management to individuals participating in the Assisted Outpatient Treatment (AOT) program who are living with serious mental health disorders as they transition back into the community from the NYS Correctional System, NYS Psychiatric Hospital System or other settings.

B. Perform duties while actively promoting an environment consistent with MHA’s Cultural Equity Policy and WELCOME Orange.

Responsibilities:

  • Attend bi-weekly Care Coordination team meetings.
  • Attend trainings and meetings relevant to AOT Care Management.
  • Will assist clients in becoming effectively and actively engaged in managing their healthcare to improve individual health outcomes while reducing avoidable hospitalizations and emergency department utilizations.
  • Conduct four monthly in person visits with clients in the community
  • Complete New York State Community Mental Health (CMH) assessments and reassessments.
  • Will provide leadership and support by facilitating and leading multidisciplinary teams of medical, mental health, chemical dependency treatment providers, courts/legal entities, social workers, nurses, representatives of the Local Government Unit (LGU) as directed by the county AOT Coordinator and other care providers toward assuring enrollees receive services as indicated in the enrollee’s single plan of care.
  • Submit monthly billing and weekly case reports for caseload.
  • Complete Care Plans, weekly status reports, and other monthly data collection information.
  • Attend monthly Orange County AOT/Forensic Care Management
  • Provide 24-hour on-call services when scheduled.
  • Maintain all client records electronically through Foothold Technology and CAIRS Electronic Record Keeping.
  • Maintain all client files according to program policies and procedures.
  • Establish and maintain linkage with appropriate community agencies and service providers.
  • Represent MHA at meetings as necessary.

Other Duties:

  • Participate in MHA’s committees as needed.

Requirements

Qualifications:

Education:

  • Bachelor’s Degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing. 

OR 

NYS licensure and current registration as a Registered Nurse and a bachelor’s degree.

OR 

A bachelor’s level education or higher in any field with five years of experience working directly with people with behavioral health diagnoses.

OR 

A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).


Experience:

  • Two years of experience (a Master’s degree in a related field may substitute for one year’s experience) 

EITHER

Providing direct services to people diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse; 

OR

Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to abroad range of services essential to successfully living in a community setting.

  • Bilingual Spanish / English a plus.
  • Effective written and oral communication skills.
  • Experience with Health Homes highly preferred.
  • Knowledge of Medicaid procedures highly preferred.
  • Experience with Electronic Health Record Program(s) required.
  • Experience working with Excel and Microsoft Word preferred.  
  • Ability to work independently with minimal direct supervision.
  • Must be organized, self-motivated and can coordinate multiple tasks simultaneously.
  • Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
  • Must have a valid driver’s license and access to a reliable, personally owned vehicle.
