Job Title: Hospice RN Case Manager


Job Summary:

We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and support for patients and their families during end-of-life care. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.


Key Responsibilities:

- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.

- Develop and implement individualized care plans in collaboration with the interdisciplinary team.

- Provide direct nursing care, including medication administration, symptom management, and wound care.

- Educate patients and families about the hospice process, available resources, and care options.

- Coordinate care with other healthcare providers, ensuring continuity and quality of care.

- Document patient progress and maintain accurate medical records in compliance with regulations.

- Participate in team meetings and contribute to quality improvement initiatives.


Skills and Qualifications:

- Current Registered Nurse (RN) license in the state of practice.

- Bachelor of Science in Nursing (BSN) preferred.

- Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care.

- Strong assessment and critical thinking skills.

- Excellent communication and interpersonal skills.

- Ability to work collaboratively in a team-oriented environment.

- Compassionate and empathetic approach to patient care.

- Knowledge of hospice regulations and standards of practice.

Requirements
  • Current, unencumbered Virginia RN license
  • One year of nursing experience, preferably in hospice, home health, or skilled nursing
  • Current Basic Life Support (CPR) certification
  • Proficient in English (spoken and written)
  • Excellent verbal and written communication skills
  • Ability to meet deadlines and work independently
  • Capable of performing CPR and airway obstruction relief
  • Able to walk or stand for at least 4 hours and climb two flights of stairs
  • Valid driver’s license with a satisfactory driving record and proof of auto insurance
  • Must complete 12 hours of continuing education, annually
