POSITION SUMMARY

The Wright Center for Community Health has an opportunity for a full-time Nurse Practitioner to join our team working in our federally qualified look-alike health center and being a part of our Street Medicine care team.  In this role you are a lifeline working alongside our internal medicine physician, you will bridge the gap between the healthcare system and our neighbors experiencing homelessness, providing high-quality primary care and addiction treatment where it is needed most.  When you are not performing work related to Street Medicine, you will work in our Scranton Health Center as part of primary care integrated healthcare team.


REPORTING RELATIONSHIPS

The position reports to the Supervising Physician for medical issues, to the Practice Manager and Co-Directors of CRNP & PA Services for practices and policies and Medical Director of the clinic. No staff report to this position. 


ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES


Clinical Responsibilities:

  • Provide full-scope primary care to the street medicine population, managing acute and chronic conditions according to established clinic standards
  • Manage complex transitions of care, ensuring patient don't fall through the cracks between the street and community services
  • Work directly with residents in our internal and family medicine residency programs on the Street Medicine rotation
  • Collaborate with residency program director to develop the didactic schedule related to Street Medicine
  • Work with the Street Medicine Care team to build and maintain strategic partnerships with community resources ands street outreach/case management teams
  • Assess health needs for children and/or adults
  • Perform and document comprehensive care and health histories and examinations on patients; Gather, interpret and integrate data to determine patient treatment plans. 
  • Work in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral
  • Collaborate with the behavioral health team
  • Interview and advise patients regarding health and illness prevention, and recommend resources to meet patient and family needs 
  • Instruct patients and families regarding medications and treatment instructions
  • Document medical records regarding all patient encounters in the office and on the telephone while staying compliant and up-to-date with the EMR process
  • Exercise HIPAA confidentiality and security measures at all times during office hours and outside of the office
  • Serve as a clinical resource for staff, clients, and families
  • Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination
  • Interpret and integrate data to determine appropriate treatment plans as needed
  • Collaborate with physicians in managing acute and long-term medical needs of patients
  • Order necessary diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
  • Interpret diagnostic test results including to assess and identify deviations from normal
  • Diagnose and treat acute, chronic, and long-term healthcare issues. Interview and advise patients regarding health and illness prevention
  • Assist in supervision of Medical Assistants and Licensed Practical Nurses
  • Effectively identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient care
  • Understand and participate in meaningful use data
  • Administer emergency care per BCLS, ACLS or PALS standards
  • Understand and adhere to “incident-to” and “non-incident-to” service and billing guidelines using proper procedures based on service conditions at the time
  • Serve as a clinical resource for staff, clients and families which includes answering triages, help streamline patient flow in office
  • Practice as an outpatient provider with thirty-two (32) hours of patient care per week (with availability for 3 patient slots per hour, 96 patient slots per week) and eight (8) hours of administrative time per week, and must be present on-site at least forty (40) hours per week, which includes administrative time. Administrative time must be coordinated and planned to occur outside of patient care hours and when there is otherwise an adequate number of providers to cover overall patient care responsibilities. The particular days scheduled will be worked out with the Medical Director and Practice Manager
  • Work some late nights, on average one to two nights per week
  • Participate in the outpatient call schedule, which typically includes one weekend per month and the occasional weekday as needed, with physician as backup
  • Participate in weekend rotation including up to two Sundays per month, or two Saturdays per month, or any combination of one Saturday and one Sunday per month, at any of Corporation’s practice locations; however, additional weekend rotation days could be scheduled from time to time based on operational and clinical needs of the Corporation as determined by Corporation in its sole discretion
  • Participate in holiday rotation, working 1-2 holidays per year. When holiday is worked Provider will have 90 days to use as a day off
  • Work additional hours/days, as needed, to cover colleague time off and staffing emergencies
  • Participate in nursing home on call coverage as needed


Requirements

REQUIRED QUALIFICATIONS

  • Licensed by the State of Pennsylvania as both a Registered Nurse (RN) and a Certified Registered Nurse Practitioner (CRNP)
  • Valid DEA
  • To write prescriptions, one must also be licensed by the State of PA for a CRNP Prescriptive Authority
  • Demonstrated ability to function both independently and in collaboration with other health care professionals 
  • Previous experience working as a nurse in home health, hospital, or other medical setting 
  • BLS, ACLS and PALS Certifications 
  • Valid PA driver’s license
