Description
Responsibilities
- Provide physical therapy services under the supervision of a licensed Physical Therapist and according to established treatment plans.
- Assist in implementing individualized physical therapy programs for people evaluated by the Physical Therapist.
- Follow and carry out treatment plans to support therapeutic goals and outcomes.
- Collaborate with physicians, therapists, caregivers, and support staff to ensure coordinated care.
- Educate and train ancillary staff and caregivers on therapy techniques and program carryover.
- Support the integration of therapy programs into individuals' daily routines and activities.
- Complete accurate clinical documentation, billing records, and required insurance-related forms in a timely manner.
- Travel to community-based locations to provide therapy services as needed.
- Reports incidents, changes or concerns promptly to the Supervising Physical Therapist and Director of Clinic Services.
- Maintain professional licensure, comply with agency policies and regulatory requirements, and protect the confidentiality of individuals served.

Requirements
- Licensed and currently registered as a Physical Therapy Assistant by the New York State Department of Education.
- A valid driver's license and good driving record acceptable to the Agency's insurance carrier.
