Sylva Valley Health & Rehab is hiring a Speech-Language Pathologist (SLP) to provide high-quality therapy services in a skilled nursing setting. This role is ideal for an SLP who enjoys functional, resident-centered care and strong collaboration with nursing, dietary, and the interdisciplinary team.

Key responsibilities

• Evaluate and treat residents for swallowing (dysphagia), communication, cognition, and voice needs
   
• Develop and implement individualized plan of care with functional goals
   
• Provide caregiver/staff education on swallowing safety, communication strategies, and recommended precautions
   
• Collaborate with nursing, dietary, and providers on diet recommendations and changes in condition
   
• Document evaluations, progress notes, and outcomes to support skilled coverage and care needs
   
• Participate in care conferences/IDT and discharge planning as needed
   

Requirements

• Current Speech-Language Pathologist license (state-licensed)
   
• Graduate of an accredited SLP program
   
• Strong documentation and communication skills
   
• SNF/rehab experience preferred (new grads considered with strong clinical skills)
