Location: Hybrid position - Required to be in our Nashville, TN office & the ability to work remote (2-3 days per week)

Status: Full-Time | Monday–Friday | 40 hours/week 

Reports to: Vice President, Clinical Operations & Palliative Care




About This Role

The Care Coordination Specialist plays an important role in supporting providers and clinical staff by managing office-based administrative and care coordination tasks. This position assists with processing prior authorizations, diagnostic results, referrals, and maintaining order follow-up. The ideal candidate is organized, detail-oriented, and comfortable working in a healthcare office setting. This role requires the ability to independently prioritize and manage a workload with competing deadlines in a fast-paced environment. Experience with back-office healthcare operations is highly valued, along with strong communication skills, reliability, and a patient-centered approach. The Care Coordination Specialist will be accountable for meeting Alive Care Standards and collaborating effectively with Alive team members as needed.

CLINICAL AND ADMINISTRATIVE SUPPORT 

  • Process new referrals and field initial interest in services in a personable and professional manner 
  • Process prior authorizations for medications, diagnostics, and durable medical equipment in a timely and accurate manner.
  • Coordinate referrals to specialists, including obtaining required documentation and insurance approvals.
  • Manage laboratory and diagnostic results, ensuring results are reviewed, documented, and communicated per provider instructions.
  • Perform order follow-up, including tracking outstanding labs, imaging, and referrals and closing loops with patients and providers.
  • Enter, update, and maintain accurate updates in the electronic medical record (EMR).
  • Prioritize daily workload to address urgent authorizations, time-sensitive results, and referral deadlines appropriately.

Requirements


EDUCATION and/or EXPERIENCE

  • High school diploma or equivalent required. Healthcare administrative professional with a minimum of 2 years of healthcare
  • Experience with EMR systems 
  • Knowledge of medical terminology
  • Experience working with Medicare and commercial insurance plans

Preferred: 

  • Experience in hospice and/or palliative care is strongly preferred. 
  • Experience preferably in outpatient healthcare settings
