Job Title: RN/LPN On-Call RN


Schedule: Monday - Thursday nights

Job overview: Luminos Hospice is seeking a skilled and compassionate On-Call RN/LPN to join our team. As an On-Call RN/LPN, you will be responsible for providing quality care to our patients during off-hours and weekends. Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.


Requirements

- Active RN/LPN license in the state of OK

- Minimum of 2 years of experience in a healthcare setting, hospice preferred

- Ability to work independently and make sound decisions

- Excellent communication and interpersonal skills

- Strong attention to detail and organizational skills

- Flexibility to work on-call shifts, including weekends and holidays

- Specialized skills preferred in palliation and end-of-life

- Reliable transportation; valid auto liability insurance and driver's license
