What You’ll Do
As the Resident Support Services Manager, you will serve as a trusted advocate and resource for residents and their families by:
- Assessing residents’ psychosocial needs and communicating findings to the care team
- Developing and implementing person-centered interventions
- Maintaining accurate and timely clinical documentation
- Connecting residents and families with community resources and support services
- Serving as a liaison between residents, families, and external agencies
- Collaborating with interdisciplinary teams to support holistic care
- Participating in care planning, case conferences, and discharge planning
- Addressing behavioral concerns to ensure safety and reduce unnecessary medication use
- Supporting gradual dose reduction through coordinated medication reviews
- Consulting with licensed social work professionals on complex cases
What You Bring
- Strong verbal, written, and organizational skills
- Knowledge of clinical social work practices, especially in geriatric care
- Ability to handle sensitive information with professionalism and confidentiality
- A collaborative mindset and genuine compassion for residents and families
Requirements
- Must meet all state-specific background check requirements prior to working in the facility
- Experience working with vulnerable or geriatric populations strongly preferred

Requirements
Education
- Associates or Bachelor’s Degree in a psychology-related field.
Licenses/Certification
- Must have a valid driver’s license.
Experience
- Six months experience in a long-term care environment preferred.
Salary Description
$25.00 - $30.00/hour
