What You’ll Do

As the Resident Support Services Manager, you will serve as a trusted advocate and resource for residents and their families by:

  • Assessing residents’ psychosocial needs and communicating findings to the care team
  • Developing and implementing person-centered interventions
  • Maintaining accurate and timely clinical documentation
  • Connecting residents and families with community resources and support services
  • Serving as a liaison between residents, families, and external agencies
  • Collaborating with interdisciplinary teams to support holistic care
  • Participating in care planning, case conferences, and discharge planning
  • Addressing behavioral concerns to ensure safety and reduce unnecessary medication use
  • Supporting gradual dose reduction through coordinated medication reviews
  • Consulting with licensed social work professionals on complex cases

 What You Bring

  • Strong verbal, written, and organizational skills
  • Knowledge of clinical social work practices, especially in geriatric care
  • Ability to handle sensitive information with professionalism and confidentiality
  • A collaborative mindset and genuine compassion for residents and families

Requirements

  • Must meet all state-specific background check requirements prior to working in the facility
  • Experience working with vulnerable or geriatric populations strongly preferred

Requirements

Education

  • Associates or Bachelor’s Degree in a psychology-related field.


Licenses/Certification

  • Must have a valid driver’s license.


Experience

  • Six months experience in a long-term care environment preferred.
Salary Description
$25.00 - $30.00/hour
