This position provides programmatic and clinical services across Home-Based Services, Adoption and Guardianship Support Services, Family Preservation Services, and Family-Centered Treatment. Responsibilities include conducting home visits and delivering treatment services, maintaining accurate documentation of all client interactions, and preparing required reports in alignment with service standards and organizational guidelines.

Major Responsibilities / Activities
- Provide structured goal-oriented, time limited therapy with clients as guided by their referral, case plan, and treatment goals.
- Provide Adoption & Guardianship Support Services as needed and as assigned.
- Provide care according to evidence based practice standards.
- Provide transportation for clients, as needed or necessary, to meet treatment goals.
- Engage in weekly staffing and address risk management areas to ensure safety of clients.
- Engage and assist the client’s participation in the treatment planning process.
- Accompany and support clients for court appearances, as requested by the referring agency.
- Demonstrate respect and cultural sensitivity to all clients and to all referral sources, and deliver services in a culturally competent fashion.
- Maintain strict confidentiality at all times to protect clients' privacy.
- Comply with required in-service / continuing education hours.
- Respect, support and integrate the company’s Tenets of Culture.
- Completion of clear, accurate, and timely documentation of 1.) all billable and non-billable contacts, 2.) monthly reports, 3.) court reports, and 4.) treatment plans.
- Attend all mandatory in-services / meetings.
- Attain / maintain CPR and First Aid certifications.

- Provide transportation for clients, as needed or necessary, to meet treatment goals.

- Engage and assist the client’s participation in the treatment planning process.
- Accompany and support clients for court appearances, as requested by the referring agency.

Travel
Travel typically includes driving within assigned region and will be determined by the needs of the clients. An estimation of 200 – 500+ miles per week may be required.
Physical and Visual Acuity Requirements
The physical requirement of this position is defined as Medium Work: Lifting up to 50 pounds frequently.
The visual acuity requirements needed to perform this position include having close visual acuity to perform activities such as: preparing and analyzing data and figures; viewing a computer terminal; and reading.

Required Education and Experience
- Master’s degree in social work, psychology, counseling, or marriage and family therapy
- Belief in the family preservation philosophy
- Knowledge of family of origin/inter-generational issues
- Knowledge of child and adult development and child abuse and neglect
- Knowledge of community resources
- 21+ years of age
Preferred Education and Experience
18+ months experience in social work, child welfare and/or adoption/guardianship
