Summary/Objective
Responsibilities include managing, mentoring, and training the enhanced care management (ECM) staff. The Population Health Care Manager establishes and maintains working relationships between the patient and provider teams at SoHum Health, Partnership Healthplan of California, and community-based organizations. This model utilizes a team approach, focused on continuous partnership among the patient, caregiver, physician, hospital, and community resources, to provide continual, coordinated care. The Population Health Care Manager will implement and oversee community health worker (CHW) and ECM programs and initiatives. It is essential that the Population Health Care Manager quickly gain and maintain an up-to-date understanding of the CHW and ECM programs.
Essential Functions
Develop and implement workflows, programs, and initiatives relevant to CHW and ECM job functions
Works with the patients/enrollees' care team to conduct comprehensive assessments and develop and update care plans for each patient/enrollee
Recognizing, prioritizing, and acting on opportunities for improvement
Networking, including developing and maintaining positive working relationships
Produce deliverable products on time, within budget, and with minimal direction
Attend conferences, courses, and educational forums as approved and complete annual education requirements, as applicable

Required Education and Experience
CHW or CHW-eligible
Bachelor’s degree in social work, population health, or other related field
Minimum of 1-3 years of experience in social services, case management, public health, or related field
Preferred Education and Experience
Master's degree in social work, population health, or other related field
