GENERAL RESPONSIBILITIES:
The Physical Therapist, working as part of a multidisciplinary team, provides direct patient care to evaluate and treat injury, disease and other conditions of health. Responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients. This position functions with a high degree of clinical autonomy, providing physical therapy services independently within the health center setting while collaborating with medical, behavioral health, and care management team members as needed. The role also includes coordinating care and clinical continuity with part-time Physical Therapy staff who provide coverage during designated sessions
SPECIFIC RESPONSIBILITIES:
- Provides skilled physical therapy treatment plans in accordance with physician orders and/or applicable referral and direct access requirements, by assessing and interpreting evaluations and test results of patients
- Formulates comprehensive treatment program by evaluating patient’s past medical history, disease/condition, impairments, disability and functional/developmental status
- Administers patient evaluation and treatments employing physical therapy modalities, procedures and interventions
- Conducts assessments in primary care settings to effectively screen and assess patients’ needs and ensure timely access to appropriate therapies.
- Prioritize patient care needs
- Completes all documentation in accordance with professional, organizational, regulatory and facility's corporate compliance standards
- Maintains written and verbal communication regarding patient and departmental concerns, and submits accurate billing information
- Coordinate with the patient’s primary physician and other medical professionals
- Participates in teaching, including annual in-service to peers
- Participates as a clinical instructor for students as assigned.
- Promotes and contributes to all infection control policies and protocols which are designed to maintain the cleanliness and readiness of all clinic and general staff areas
- Participates with departmental quality improvement activities
- Demonstrates excellence in both internal and external customer service.
- Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality
- Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH).
- Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.
- Completes other duties as assigned
MINIMUM QUALIFICATIONS:
Graduate of an accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) degree. Candidates licensed in New York State prior to the DPT requirement may hold a Bachelor’s or Master’s degree in Physical Therapy. Current NYS Physical Therapy license and registration. At least three (3) years PT experience. Current Basic Life Support (BLS) certification. Effective oral and written communication skills. Proficient in using Microsoft Office and electronic health records.
PREFERRED QUALIFICATIONS:
Five (5) or more years of experience preferred due to the independent and lead nature of the role.

Salary range: $80,000 - $100,000 annually
