ABOUT THE ROLE
Integrity Pulmonary Specialist is seeking a dedicated and compassionate Nurse Practitioner to join our growing team. In this full-time position, you will serve as a vital extension of our pulmonology practice by conducting pulmonary consultations at post-acute rehabilitation and skilled nursing facilities across the East Valley, Central Scottsdale, and Central Phoenix. Your work will be central to our mission: delivering expert, personalized pulmonary care bedside—reducing unnecessary hospital transfers and improving outcomes for our most vulnerable patients.
You will work in close collaboration with our supervising pulmonologist, who manages ventilator-dependent patients at separate facilities throughout the valley. The nurse practitioner and pulmonologist operate at distinct locations—the NP’s assigned rehabs do not include vented patient facilities—creating a well-organized, specialized care model that allows each provider to focus on their respective patient populations.
KEY RESPONSIBILITIES
• Conduct pulmonary consultations and follow-up visits at assigned post-acute rehab and rehabilitation facilities across the East Valley, Central Scottsdale, and Central Phoenix
• Perform comprehensive pulmonary assessments to identify and address patient needs before conditions escalate to hospital-level care
• Develop, implement, and manage individualized pulmonary care plans in coordination with the supervising pulmonologist
• Order and interpret diagnostic studies including pulmonary function tests, chest imaging, and lab work as appropriate
• Prescribe and manage medications related to pulmonary conditions including COPD, asthma, pulmonary fibrosis, and respiratory infections
• Communicate clinical findings and care plans with facility nursing staff, attending physicians, and multidisciplinary teams
• Document patient encounters accurately and in a timely manner in accordance with regulatory and compliance standards
• Serve as a clinical resource for facility staff regarding respiratory care best practices
• Participate in structured onboarding and clinical training provided by our supervising pulmonologist and the outgoing nurse practitioner
• Proactively identify high-risk patients and coordinate preventive interventions to reduce avoidable hospital transfers
TRAINING & ONBOARDING
We are committed to setting our new team member up for success. This position includes a comprehensive onboarding program consisting of:
• Direct mentorship from our supervising pulmonologist, who brings extensive experience managing complex and ventilator-dependent pulmonary patients
• Hands-on transition training with the current nurse practitioner who is stepping down from this role, ensuring continuity of patient relationships and facility familiarity
• Facility introductions and orientation to the clinical workflows, staff teams, and patient populations at each assigned rehab location
QUALIFICATIONS
Required:
• Active and unrestricted Nurse Practitioner license in the state of Arizona
• Master’s or Doctoral degree in Nursing (MSN, DNP, or equivalent)
• Current DEA license or eligibility to obtain
• BLS and ACLS certification
• Strong clinical assessment skills with the ability to work independently in a mobile, multi-facility setting
• Excellent communication, critical thinking, and documentation skills
Preferred:
• Prior experience in pulmonology, critical care, internal medicine, or post-acute/long-term care settings
• Familiarity with respiratory conditions including COPD, asthma, pulmonary fibrosis, sleep-disordered breathing, and respiratory failure
• Experience working in skilled nursing facilities (SNFs), long-term acute care (LTAC), or inpatient rehabilitation settings
• Bilingual (English/Spanish) a plus

WORK ENVIRONMENT
This is a field-based, mobile position. The nurse practitioner will travel between multiple post-acute rehabilitation and skilled nursing facilities across the East Valley, Central Scottsdale, and Central Phoenix. To minimize drive time and maximize efficiency, facility visits are scheduled on a staggered geographic rotation—alternating days are dedicated to one side of the valley, so you are never criss-crossing the metro area in a single day. A valid driver’s license and reliable transportation are required. The NP’s assigned facilities are entirely separate from those covered by the supervising pulmonologist; the pulmonologist manages ventilator-dependent patients at distinct locations, meaning this role does not involve vented patient care or hospital-based rounding.
