Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking a Primary Care Physician in Peoria, Illinois. This is a full-time outpatient opportunity within a value-based care model focused on delivering high-quality, patient-centered outcomes for adult and geriatric populations.

POSITION OVERVIEW
The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. This model allows physicians to focus on quality of care, prevention, and long-term patient relationships rather than high patient volume.

SCHEDULE
• Monday through Friday
• No traditional call responsibilities
• Structured outpatient clinic setting

COMPENSATION
• Base Salary Range: $174,070 – $374,920
• Quarterly bonus based on quality metrics
• Compensation aligned with experience, location, and outcomes performance

KEY RESPONSIBILITIES
• Provide comprehensive primary care to adult and geriatric patients
• Diagnose and manage acute and chronic conditions
• Develop and execute individualized care plans
• Collaborate with interdisciplinary care teams
• Maintain accurate and timely EMR documentation
• Focus on preventive care and reducing hospital admissions
• Drive patient engagement and long-term health outcomes

CARE MODEL
• Outpatient primary care focused on Medicare population
• Value-based care structure emphasizing outcomes
• Team-based interdisciplinary approach
• Enhanced patient access and continuity of care

• MD or DO required
• Board Certification or Board Eligibility in Internal Medicine or Family Medicine required
• Active, unrestricted state medical license required
• Active DEA license required
• U.S. work authorization required

• Experience in outpatient primary care preferred
• Experience with geriatric populations preferred
• Value-based care experience preferred but not required
• Strong collaboration and team-based care mindset
• Ability to manage patient panels and drive outcomes
