JOB SUMMARY: 

Evaluates residents’ needs, condition, and care and assists the care plan coordinator in developing nursing care plans for individual residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

(Other duties may be assigned.)

§ Carries out administrative and resident care duties as directed.

§ Periodically reviews ECC service plans with ECC supervisor and other health team members. Attends service plan meetings as needed.

§ Completes monthly summaries on each resident. 

§ Takes and transcribes physicians’ orders to resident medication sheet, treatment sheets, diet list, etc. in consultation with DON

§ Admits new residents by observing their needs and reviewing physician orders.

§ Sends diet orders to dietary department.

§ Assembles resident’s charts, attaching required forms.

§ Requisitions supplies, equipment, linens, emergency drugs, etc. to meet unit needs. Sees that equipment is in good working order, in consultation with DON and Administrator.

§ Evaluate/reprimand C.N.A.’s as needed.

§ Perform physical assessment on ALF and independent residents per physician order and/or following incidents or activation of emergency call light.

§ Abide by department policies and procedures and interprets them to personnel, residents, medical staff and public.

§ Gives/receives the nursing report upon report as well as counting of Narcotics upon beginning and ending shift duty hours.

§ Answers telephone inquires.

§ Order prescribed medications, supplies, and equipment as necessary, in accordance with established ALF policies.

§ Requisition and arranges for diagnostic and therapeutic services as ordered by ALF resident’s physician.

§ Completes unit or medical chart forms as required.

§ Assist in scheduling appointments and arrangements for transportation of ALF residents to other departments or facilities and clinics for diagnostic, therapeutic, dental or medical services in consultation with DON

§ Discharge or transfer residents, making necessary arrangements, and completing necessary forms per facility policy, re: giving discharge instructions to the resident or responsible party; completing discharge nurses summary; disposing the meds in an appropriate manner.

§ May assist in on-the-job orientation, instruction and supervision of nursing personnel.

§ Maintain a professional attitude at all times on the unit and a professional approach towards nursing care and personnel.

§ Notify ALF Administrator when a resident is transferred to another facility, discharged, or expires. 

§ Assure that established infection control and universal precautions are maintained when performing duties.

§ Follow up with new orders, labs, x-ray, or other diagnostic exams done on a day to day basis.

§ Prefill medication box for ALF residents and/or administer medications per physician orders.

§ Assist with resident transfers from one level of care to another within the OLT complex.

§ Documentation of all activities, writing of incident reports, and subsequent follow-up as needed, and providing a detailed report at the end of each shift for the next nurse to be aware of situations that may occur and have occurred. 

§ Performs other duties as assigned.

REQUIREMENTS, SKILLS, ABILITIES AND OTHER ATTRIBUTES:

§ Must be a Licensed Practical Nurse in the State of Florida.

§ Must have HIV/AIDS training per state guidelines.

§ Familiar with and abides by ALF and CCRC regulations.

§ Must have ECC training or be available to attend classes.

§ Ability to read, write and speak the English language in an understandable manner and to be able to carefully follow written and oral instructions.

§ Ability to sit, stand, bend, lift and move intermittently throughout the workday.

§ Maintain good general health and demonstrate emotional stability.

§ Ability to manage and cope with the mental and emotional stress of the position accordingly.

§ Work closely with ill, disabled, elderly, and emotionally upset people.

§ Assume the responsibility for maintaining professional competence through participation in programs, seminars, and/or training. 

§ Willing to work beyond the normal working hours, on weekends, and in other positions temporarily, when necessary.

§ Ability to handle department equipment according to standard procedures.

§ Ability to work effectively with residents, staff, and visitors with flexibility, personal integrity, sensitivity, and discretion at all times. 

§ Reports all accidents and incidents to supervisor no matter how minor they may be.

§ Attends and participates in in-service classes, on-the-job programs, etc. as scheduled and directed.

§ Honor the Residents’ Rights polices at all times.

§ Maintains and protect the privacy and confidentiality of protected health information (PHI) per the company’s HIPAA medical privacy policy. 

§ Be expected (and required) to perform any task in a safe, sanitary and efficient manner in accordance with established procedures.

§ Follows facility safety program.

§ Assumes accountability for data contained in the Employee Handbook.

§ Any exposure to blood or body fluids that contain visible blood must be reported to supervisor on the shift in which the incident occurs. 

§ Be familiar with universal precautions (i.e. frequent hand washing, using gloves, etc.).

§ Understand that this position contains Category I tasks that may involve exposure to blood, body fluids, or tissues. 

§ Ability to understand and use universal precautions and appropriate protective measures, which are readily available to every employee, engaged in Category I tasks.

§ Ability to perform all functions of the job classification without posing a direct threat to the health or safety of other individuals in the workplace.
