Summary:
The Intake Navigator is responsible for the end-to-end coordination and management of patient referrals within the Electronic Health Record (EHR) system. This role serves as a primary point of contact for electronic referral workflows, ensuring referrals from both internal and external providers are efficiently retrieved, reviewed, and processed. The Intake Navigator supports seamless transitions of care by collaborating closely with clinical teams and specialists while managing referral activity within the EHR environment.
Key Responsibilites
- EHR Referral Management: Monitor, retrieve, and process referrals from EHR work queues, electronic referral portals, and in-basket messages in a timely manner.
- Intake & Review: Evaluate incoming referrals for completeness and accuracy, ensuring all required documentation (e.g., demographics, clinical notes, discharge summary/orders) is included before acceptance.
- Insurance Verification: Perform insurance eligibility checks, confirm in-network status, and obtain necessary prior authorizations to support referral processing.
- Clinical Liaison: Collaborate with clinical staff to redirect referrals that do not meet criteria and escalate complex or unclear cases to providers for further review.
- Data Accuracy & Documentation: Accurately upload, scan, and attach referral documentation into the EHR and intake systems to ensure compliance, completeness, and audit readiness.
- Communication & Coordination: Serve as a liaison between referring sources/providers, specialty offices, and patients to confirm referral receipt, communicate status updates, and assist with appointment scheduling.
- Closing the Loop: Ensure referrals are properly documented and closed within the EHR, including obtaining and attaching specialist consultation notes when applicable.
Requirements
Qualifications:
- High School Diploma or GED required
- Certified Medical Assistant (CMA) or related certification preferred
- EHR Experience: Minimum of 1–2 years of experience with EMR/EHR systems (e.g., Epic, Careport, Hospital and Home Care Based systems, etc).
- Medical Knowledge: Strong understanding of medical terminology and familiarity with referral and insurance processes.
- Technical Skills: Proficiency in Google Workspace applications (Gmail, Google Chat, Sheets, Docs) and experience managing electronic hospital and home care-based referral portals.
- Attention to Detail: High level of accuracy in data entry, documentation, and insurance verification processes.
- Communication Skills: Excellent verbal and written communication skills, with the ability to effectively interact with patients, providers, and healthcare staff.
Salary Description
$20 - $21 per hour
