JOB SUMMARY
The Licensed Integrated Behavioral Health Provider is responsible for providing outpatient mental health resources for patients within the primary care setting. The Integrated Behavioral Health provider will work collaboratively with an interdisciplinary team of providers, including, primary care MD’s, NP’s, PA’s, Nurses, clinical pharmacists, medical assistants, and psychiatric providers. Responsibilities of this role include short term mental health counseling to Appalachian Mountain Health patients to help them build resiliency and internal resources to cope with stressors in their lives.
KEY ACCOUNTABILITY #1
- Work collaboratively within a team of medical and mental health providers. Must maintain open communication and accessibility to encourage mental health integration in the clinic setting.
- Initiate behavioral health care services with patients through introductions to care, conducting a diagnostic assessment through interviews with patients to determine behavioral health needs, address symptoms of concern, complete risk assessments, and develop appropriate response plans for patients, specific to their behavioral health concerns.
- Work with a variety of patients with a wide range of mental health diagnoses, and lived experiences, including substance use disorders.
- Provides consultation services for mental health recommendations to patient’s multidisciplinary team of providers.
- Initiates treatment interventions; coping tools, individual counseling, and family counseling and crisis interventions as appropriate.
KEY ACCOUNTABILITY #2
- Support patients through challenges with an emphasis on building sense of internal locus of control.
- Document every patient interaction, and outcomes, including referrals
- Provide patient education as it relates to their mental health needs.
- Charts and maintains notes within 72 hours in compliance with CMS documentation standards.
- Protects confidentiality and the abide by HIPPA Laws as well as 42 CFR.
EDUCATION/EXPERIENCE
- Master's Degree in Social Work
- Master’s Degree in Mental Health Counseling
PERFORMANCE REQUIREMENTS
Knowledge and Credential:
- 3+ years in a Clinical Setting
- Licensed Clinical Social Worker (LCSW, LCSWA)
- Licensed Clinical Mental Health Counselor (LCMHC, LCMHCA)
- Licensed Psychologist (LPA, PsyD)
Skills:
- Experience with behavioral health screening tools and risk assessments
- Experience with a wide range of patient populations, all ages and stages preferred.
- Experience with substance use treatment and willingness to work with populations experience substance use disorders.
- Training in brief, evidence-based treatment models
- Familiarity with population health and team-based care models
- Abilities:
- Strong clinical assessment and brief intervention skills
- Ability to work in a fast-paced, team-based medical environment
- Excellent communication and collaboration skills
- Flexibility and adaptability in a dynamic care setting
- Cultural humility and trauma-informed approach
PHYSICAL REQUIREMENTS
- Seeing: Must be able to read written and printed material, as well as use computer, fax machine, Xerox machine and other office equipment.
- Hearing: Must be able to hear well enough to communicate with coworkers and others. Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound.
- Sitting: Must be able to sit for the majority of the work shift while answering telephone and communicating with others.
- Standing/Walking/Mobility: Must be able to stand intermittently and be capable of walking on hardwood and linoleum floors with concrete underpayments. Must be able to walk between various clinical areas and departments.
- Fingering/Grasping/Feeling: This position requires manual and finger dexterity and eye-hand coordination for easy and skillful use of hands when working with and performing normal office duties and responsibilities.
