Position Summary

Bradbury Commons is seeking a compassionate and motivated Full-Time Physical Therapist to join our interdisciplinary care team in a long-term care setting. This role is focused on helping residents maintain or improve mobility, function, and overall quality of life through individualized, evidence-based therapy programs.

The ideal candidate is patient-centered, organized, and comfortable working with geriatric populations, including those with chronic conditions, post-surgical needs, or mobility limitations.

Key Responsibilities

  • Conduct comprehensive evaluations of residents to assess physical function, mobility, strength, balance, and endurance 
  • Develop individualized treatment plans based on clinical findings, physician orders, and resident goals 
  • Provide skilled physical therapy interventions, including therapeutic exercise, gait training, transfer training, balance activities, and pain management techniques 
  • Monitor resident progress and modify treatment plans as needed to ensure optimal outcomes 
  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other team members to coordinate care 
  • Educate residents and families on treatment plans, safety strategies, and home exercise programs 
  • Ensure all documentation is accurate, timely, and compliant with state, federal, and facility guidelines 
  • Participate in care planning meetings and contribute to interdisciplinary team discussions 
  • Maintain a safe and clean therapy environment and adhere to infection control standards 
  • Support facility goals related to quality outcomes, resident satisfaction, and regulatory compliance 

Qualifications

  • Graduate of an accredited Physical Therapy program (DPT or equivalent) 
  • Current, active Physical Therapist license in the state of Oklahoma (or eligibility to obtain) 
  • Previous experience in a long-term care or skilled nursing setting preferred 
  • Strong clinical reasoning and problem-solving skills 
  • Ability to build rapport with residents and families 
  • Excellent communication and documentation skills 
  • Ability to work independently while also contributing to a team environment
