The YoloCares Clinical Team
The Registered Nurse (RN) Case Manager will join a highly skilled, compassionate interdisciplinary clinical team dedicated to providing patient- and family-centered hospice care. YoloCares’ clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who work collaboratively to support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care.
Position Summary
YoloCares is seeking an experienced and innovative RN Case Manager focused on collaboration, relationship management, and education to our patients in Solano County. We are looking for a full-time dedicated RN Case Manager to provide skilled nursing care in the Solano County.
Key Responsibilities
- Complete the nursing assessment of assigned patients. Maintain an ongoing assessment within hospice guidelines to determine if the patient fulfills hospice criteria.
- Collaborate with other core disciplines (medicine, social work, and spiritual care counseling) in accordance with the Plan of Care to meet care goals.
- Coordinate the provision of necessary medications, durable medical equipment, and disposable medical supplies. Ensure cost-effective and timely provision of the above.
- Know and apply Universal Precautions in the provision of all direct patient care.
- Provide ongoing professional nursing care for each assigned patient. Regularly assess and reevaluate patient nursing needs and related patient and family support needs. Report findings to supervisors and the interdisciplinary team (IDT), as appropriate.
- As patient and family needs or goals are assessed and/or change: (1) communicate with the Attending Physician or designee regarding changes in condition and emergent patient needs, and obtain and document necessary physician orders; and (2) assess and collaborate with supervisor on appropriate change in level of care for patients, including Continuous Care, Respite Care, General In-Patient Care, or decertification/discharge from the Hospice Program.
- Provide instructions regarding medication, activity, diet, symptom management, safety, expected disease progression, and the dying process in accordance with the patient’s Plan of Care with Attending Physician input.
- Ensure all patient documentation is complete, accurate, and reflects nursing assessments, interventions, and goals according to the individualized patient and family Plan of Care and YoloCares Standards of Care within 24 hours of patient visit.
- Complete or update patient Plan of Care so problems, plans, actions, and goals are accurately and clearly stated and written in accordance with federal, state, and YoloCares standards. Medical orders are obtained, documented, and submitted promptly as Plan of Care amendments. Visit notes are written to clearly reflect interventions and goals.
Qualifications
- Current, active Registered Nurse (RN) license in the State of California.
- Ability to communicate clearly and effectively in English, both verbally and in writing; Spanish language skills are a plus.
- Minimum of two (2) years of recent clinical nursing experience preferred; experience in home care or hospice care highly desirable.
- Demonstrated clinical nursing skills and competence, with an understanding of and commitment to the philosophy, principles, and goals of hospice care.
Pay range: $114,000-$124,800 Annually
