Job Title: Certified Nursing Assistant (CNA) / Home Health Aide (HHA)
Supervised By: Director of Clinical Services / Registered Nurse Case Manager

Position Summary:
Provides personal care and supportive services to patients in their place of residence under the direction and supervision of a Registered Nurse, in accordance with physician-ordered plan of care, agency policies, State and Federal regulations and accreditation standards. Services are delivered in a manner that promotes patient safety, dignity, infection prevention and optimal outcomes.
Essential Duties and Responsibilities:
Plan of Care Implementation
- Delivers care strictly as outlined in the physician-ordered and RN-managed care plan.
- Verifies assigned tasks prior to each visit and reports inability to perform ordered services.
- Supports patient independence while encouraging safe participation in self-care.
Personal Care Services
- Provides assistance with activities of daily living, including:
- Bathing, grooming, oral hygiene, skin and nail care per scope of practice
- Dressing and undressing
- Toileting and continence care and grooming assistance
- Feeding and hydration assistance
- Transfers, positioning, pressure injury prevention and ambulation assistance
- Utilizes proper body mechanics and fall-prevention techniques at all times.
Observation and Reporting
- Observes and reports changes in:
- Physical condition, pain level, skin integrity, or functional status
- Cognitive or psychosocial status
- Home safety concerns or environmental risks
- Immediately reports significant findings to the supervising RN.
- Recognizes and reports suspected abuse, neglect or exploitation per agency policy.
Vital Signs and Infection Control
- Obtains/reports and records vital signs when assigned and within scope of practice.
- Follows infection prevention and control standards, including hand hygiene and PPE use.
- Maintains a safe care environment in the patient’s home.
Documentation
- Completes accurate, objective and timely documentation in the EMR within Agency deadlines.
- Communicates patient observations to licensed staff to support care coordination.
- Complies with supervisory visit requirements or as required by regulation.
Compliance, Quality and Safety
- Adheres to all applicable:
- State and Federal regulations
- Accreditation standards
- Agency policies and procedures
- Participates in Quality Assessment and Performance Improvement (QAPI) activities.
- Demonstrates accountability for patient safety and risk reduction.
Professional Responsibilities
- Maintains patient confidentiality and HIPAA compliance at all times.
- Complies with all organizational policies, procedures and ethical business practices.
- Completes all required education, training, evaluations and maintains current knowledge and competency through continuing education.
- Completes required in-services, competencies and annual performance evaluations.
- Reports to work on time and completes assigned tasks within designated timeframes.
- Adheres to agency dress code and professional appearance.
Qualifications and Regulatory Requirements
- Maintains an active CNA certification in accordance with state requirements.
- Completes:
- Initial orientation and competency validation
- Ongoing in-service education
- Attends at least 80% of in-services/meetings
- Maintains a minimum of 12 hours of annual continuing education, or as required by:
- State and Federal regulations
- Accreditation standards
- Agency policy
- High school diploma or equivalent preferred.
- One year of experience within the last two years as a CNA or HHA in:
- Home health
- Skilled nursing facility
- Hospital
- Graduation from a State-approved CNA program within the last two years.
Skills and Competencies:
- Knowledge of CNA scope of practice.
- Ability to follow written and verbal instructions.
- Basic computer and EMR documentation skills.
- Effective communication and observational skills.
- Ability to prioritize tasks and manage time effectively.
Physical and Environmental Demands:
- Ability to stand, walk, bend and lift for extended periods.
- Ability to lift, push and move up to 50 pounds.
- Ability to assist with patient transfers and mobility.
- Manual dexterity and hand-eye coordination required to perform assigned duties.
- Ability to safely operate durable medical equipment in the home.
- Ability to communicate effectively in English, both verbally and in writing. Communication in additional languages is preferred.
