Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Nurse Practitioner in Ferndale, California.

POSITION OVERVIEW
• Mission-driven outpatient Family Nurse Practitioner opportunity serving diverse and underserved populations along California’s Northern Coast
• Join a collaborative, integrated care model with behavioral health, telepsychiatry, chiropractic, and specialty consult support
• Practice evidence-based medicine with strong staffing support and structured workflows designed to improve provider balance and patient outcomes

SCHEDULE & PRACTICE MODEL
• Outpatient-only Family Practice setting
• Flexible full-time scheduling options available
• 5x8 or 4x10 shift options
• Integrated interdisciplinary care environment
• Panel-based primary care model with strong support structure

CLINICAL RESPONSIBILITIES
• Provide comprehensive outpatient primary care to patients across the lifespan
• Manage chronic and complex medical conditions with interdisciplinary support
• Collaborate with dedicated MAs, RN support, behavioral health teams, and virtual triage services
• Participate in quality improvement initiatives and collaborative care planning
• Utilize integrated support resources including telepsychiatry and specialty consult services

COMPENSATION & BENEFITS
• Base salary: $148,627.20 – $168,553.60 annually
• $12,400 annual access bonus for meeting patient access targets
• Up to $44,000 annual production bonus potential
• Sign-on and retention bonus structure for partnership-eligible providers
• Alternative sign-on and retention structure for non-partnership track providers
• $10,000 relocation reimbursement
• Additional $10,000 relocation support for qualifying family relocation
• 29 days paid time off including CME, holidays, and sick time
• Comprehensive medical, dental, and vision insurance
• NHSC and federal loan repayment eligibility
• $5,000 annual professional expense card for licensing, DEA, CE, and equipment
• Laptop and cell phone provided

Requirements

• Board Certified Family Nurse Practitioner REQUIRED
• Active California Nurse Practitioner license or ability to obtain REQUIRED
• DEA license REQUIRED or ability to obtain
• Outpatient primary care experience preferred
• Mission-driven approach to community healthcare REQUIRED
• Comfortable treating patients across the lifespan REQUIRED
• Team-based and collaborative clinical mindset REQUIRED
• Rural health or underserved community interest strongly preferred
