SUMMARY:
The Community Health Nurse promotes good health practices, coordinates existing health resources, and provides direct health care services to Tribal members in the CTCLUSI service delivery area.
PRINCIPAL ACTIVITIES & RESPONSIBILITIES:
- Provides therapeutic and preventive nursing care to individuals, groups, and the Tribal community.
- Provides public health nursing care in the home, with an emphasis on prevention, chronic disease control, communicable disease control, and nursing assessments, especially to high-risk patients such as Elders and those with serious medical conditions.
- Works as part of the health team to coordinate activities and interventions of other Tribal, State, and Federal health and social service agencies to assist Tribal members and their families.
- Communicates with Tribal, IHS, and other resources in addressing community health needs.
- Provides guidance and education for Community Health Aides in prevention, disease management, and other applicable health topics.
- Participates, as available, in Tribal activities, such as Tribal Family Gatherings, youth events, youth camps and other events.
- Works together with Diabetes Educator to educate Community Health Representatives regarding diabetes education and prevention.
- Prepares articles and flyers for publication in the monthly Tribal newsletter.
- Participate in emergency preparedness and response efforts, including providing healthcare services during crises like natural disasters or outbreaks
- Provides blood glucose testing at events. Maintain and manage blood glucose monitors, testing supplies and educational materials for distribution to Tribal members.
- Maintains strict confidentiality in all job duties.
- Interacts with Tribal members and the general public in a courteous, professional, and efficient manner.
- Other duties as directed by management.
LEVEL OF AUTHORITY & RESTRICTIONS:
- Supervises a small team or group of individuals, with some authority in decision-making and task delegation.
PHYSICAL & MENTAL DEMANDS:
- Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
- Vision abilities required by this job include close vision and the ability to adjust focus.
- Must be required to push, pull, lift, and/or carry up to 40 pounds.
WORKING CONDITIONS & ENVIRONMENT:
- Wok is performed in an office setting.
- Moderate noise level with frequent interruptions and distractions.
- Must be willing and able to travel both locally and within the CTCLUSI service delivery area.
- May require local and out of town/overnight travel.
- May be required to attend Tribal youth camps and other activities.
Requirements
MINIMUM JOB REQUIREMENTS:
- Must be 25 years of age or older per our vehicle use policy.
- Must have a valid driver’s license and be able to be insured to drive Tribal Government vehicles.
- Registered nurse with BSN Degree and training in Community Health or Public Nursing required.
- Must possess a current unencumbered Oregon Board of Nursing license and be eligible for renewal.
- 3 years’ experience providing services both in the community and within the patients’ home.
- Possession of a valid Food Handler’s card is required within 90 days of hire. Ongoing recertification must be maintained in accordance with regulatory requirements.
- First Aid and CPR certification must be obtained within 90 days of hire, or as soon as training becomes available if not offered within that timeframe. Recertification must be maintained as required.
- Advanced experience and proficiency in the use of Microsoft products (Excel, Outlook, PowerPoint, Word, etc).
- Must be highly organized, self-motivated, and capable of managing complex projects while independently fulfilling all job responsibilities with minimal supervision.
- Ability to present in a public forum (online and in-person) to large and small groups of people.
