Remote RPM Alerts Care Coordinator (LPN/LVN) 

TimeDoc Health is seeking compassionate and patient-focused LPN to join our RPM Alerts Team and help patients with chronic conditions stay healthier and out of the hospital.

What You'll Do

• Respond to Remote Patient Monitoring (RPM) alerts and conduct patient outreach
• Support patients with chronic conditions through Chronic Care Management (CCM) and RPM services
• Educate patients about their health conditions and help them achieve their care goals
• Coordinate with providers, office staff, caregivers, and community resources to ensure patients receive the support they need
• Document patient interactions, update care plans, and identify barriers to care

This is a phone-based care coordination role, with team members spending the majority of their day speaking with patients and documenting care.

Qualifications

• Active Multi-State LPN License required
• Minimum 1 year of LPN experience
• Bilingual (English/Spanish) preferred
• Strong communication, documentation, and critical thinking skills
• Care coordination, case management, call center, triage, or geriatric experience is a plus

Schedule Requirements

• 8–29 hours per week
• Some weekdays evenings available 
• 3 out of 4 weekends on average 
• Flexible 4-hour shift options
• Must work at least 2 major holidays per year

Remote Work Requirements

• Private home office with minimal distractions
• Second monitor required
• Reliable internet with upload speeds of at least 100 Mbps
• Company laptop and software provided
