Description

In-Home Independent Support shall provide short-term, intermittent services to enable participants to achieve their goals as outlined in their Person-Centered Service Plan (PCSP). Services may include hands-on assistance, guidance, observation, and training in activities of daily living, instrumental activities of daily living, and community participation. Services are provided in the participant’s own residence or family home and may be participant-directed in accordance with Section 10 of the administrative regulation.  

Requirements

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Provide hands-on assistance or guidance according to participant’s functional needs. 
  • Assist participants with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). 
  • Support participants in medical care management, including appointments and transportation as authorized. 
  • Ensure compliance with all agency policies, regulations, and PCSP objectives. 
  • Provide supervision and support in community settings as identified in PCSP. 
  • Maintain a safe, clean, and healthy environment for participants. 
  • Observe and report changes in health or behavior to supervisor. 
  • Administer behavior support or interventions according to approved plans. 
  • Complete all documentation timely, including service notes, time logs, and incident reports. 
  • Attend mandatory trainings, team meetings, and work scheduled overtime as needed. 
  • Maintain professionalism, respect, confidentiality, and team collaboration. 
  • Perform other duties as assigned. 

QUALIFICATION REQUIREMENTS: 

  • High school diploma, GED, or age 18 and above. 
  • Ability to read, comprehend, and follow instructions. 
  • Effective communication skills for one-on-one and small group interactions. 
  • Basic math skills to calculate hours, dosages, or measurements. 
  • Ability to apply common sense and problem-solving to participant needs.
