At 360 Total Care, we believe meaningful days happen in the community, not inside four walls. We are looking for energetic, compassionate, and dependable professionals who want to help adults build friendships, develop independence, and experience everything their community has to offer.

This is not a traditional day program position. Our team spends most of the day in the community participating in volunteer opportunities, recreational activities, educational experiences, fitness programs, and social events.

What Success Looks Like

A successful day starts by safely picking up participants, supporting them throughout multiple community activities, encouraging engagement and personal growth, and ending the day by safely returning everyone home.

Our goal is simple: help people live fuller, more connected lives through meaningful community experiences.

Position Overview

As a Community Integration Specialist, you will be responsible for a small group of up to four individuals throughout the day. You will serve as both a driver and direct support professional, ensuring participants are safe, engaged, respected, and actively involved in their communities.

This position requires professionalism, reliability, patience, strong communication skills, and a genuine passion for helping others succeed.

Primary Responsibilities

Transportation & Safety

  • Safely transport participants to and from their homes and community activities
  • Conduct vehicle safety checks and maintain a clean vehicle
  • Follow all traffic laws and company transportation policies
  • Ensure participant safety during transportation and community outings

Community Integration

  • Lead and participate in community-based activities
  • Encourage social interaction and relationship-building
  • Support individuals in achieving personal goals and increasing independence
  • Assist participants with community access, volunteering, recreation, fitness, and educational opportunities
  • Promote dignity, inclusion, and person-centered supports


Requirements

Professional Responsibilities

  • Maintain a positive and professional attitude at all times
  • Represent 360 Total Care professionally within the community
  • Complete required documentation accurately and timely
  • Communicate effectively with supervisors, families, and team members
  • Follow all DODD and company policies and procedures

Qualifications

Required:

  • Valid driver's license with acceptable driving record
  • High school diploma or GED
  • Ability to pass background checks
  • Ability to lift, assist, and support participants as needed
  • Strong communication and interpersonal skills

Must Have or Be Willing to Obtain:

  • CPR Certification
  • First Aid Certification
  • Medication Administration Certification
