Licensed Physical Therapist (PT) – Skilled Nursing Facility

HTS is seeking a dedicated and licensed Physical Therapist (PT) to join our dynamic therapy team at a highly-rated skilled nursing facility in Anderson, Indiana. This is an excellent opportunity for a professional who thrives in a fast-paced rehab environment and values collaborative teamwork. Our facility offers a spacious, well-equipped therapy gym with specialized areas such as a training kitchen and bathroom to support comprehensive rehab-to-home practices. The position is PRN, providing flexibility to support the team during planned time off, caseload increases, and weekend needs.
Additional opportunities are available at our facilities in Elwood and Summitville.
Temporary 12 week assignment starting sometime in August which could provide a consistent 30 to 35 hours per week.
Job Details:
- Conduct thorough evaluations of patients to determine appropriate treatment plans
- Perform recertification assessments and document progress notes accurately
- Develop and implement individualized therapy plans to meet patient goals
- Manage discharges and ensure smooth transitions for patients returning home or to other care settings
- Support the therapy team during peak times, including weekends and staff absences
- Collaborate with interdisciplinary team members to optimize patient outcomes
- Maintain compliance with all regulatory and safety standards

Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include graduation from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, subsequent passing of a state-administered national exam, and fulfilling the state’s requirements for licensure.
Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
