As a Service Coordinator for the Elderly and Persons with Disabilities, you will be responsible for ensuring that our tenants receive the necessary services and support to help them live independently and with dignity. Your role will be to coordinate services and resources to support our tenants' well-being and independence. You will work closely with tenants, families, caregivers, and service providers to ensure that all needs are met and that services are delivered efficiently. Responsibilities:

- Conduct assessments to identify tenants' needs and develop service plans. - Coordinate with service providers to ensure services are delivered efficiently and effectively. - Monitor progress and make necessary adjustments to service plans. - Advocate for tenants to ensure they receive the services and support they need. - Provide education and support to tenants, families, and caregivers. - Maintain accurate and up-to-date records of tenant services and progress. - Collaborate with other professionals to provide comprehensive care to tenants. 


Requirements

 Bachelor's degree in social work, gerontology, or a related field is preferred, however may be substituted with experience. - Experience working with elderly and persons with disabilities. - Knowledge of community resources and service providers. - Strong communication and interpersonal skills. - Ability to work independently and as part of a team. - Strong organizational and time management skills. - Compassionate and empathetic approach to working with our tenants.
