Position Description: 

The Direct Service Provider (DSP) delivers person-centered Home and Community-Based Services (HCBS) under the Kentucky 1915(i) RISE Initiative. The DSP is responsible for implementing the specific services identified in the participant’s Person-Centered Service Plan (PCSP), focusing on skill development, community integration, and health and safety monitoring. 


Responsibilities:  

  • Service Delivery: Deliver services strictly in accordance with the participant’s individualized service plan (PCSP). 
  • Participant Support: Promote autonomy, dignity, and informed choice while supporting community integration consistent with HCBS requirements. 
  • Safety Monitoring: Actively monitor participant health, safety, and welfare during service delivery; report incidents per agency policy and regulatory requirements. 
  • Documentation: Complete accurate, professional, and timely documentation for every service encounter. 
  • Professionalism: Maintain strict professional boundaries and HIPAA-compliant confidentiality. 
  • Field Work: Travel within your assigned region to meet with participants face-to-face in their homes or other community locations. 
  • Collaboration: Participate in required supervision, team meetings, and training sessions. 
  • Service Versatility: DSPs may be required to deliver multiple supported services, including: 
  • In-Home Independent Living Supports 
  • Supported Employment & Education 
  • Planned Respite 
  • Housing and Tenancy Supports 
  • Non-Medical Transportation 
  • Supervised Residential Care & Assistive Technology 
Requirements

 

Education & Experience: 

  • High School Diploma or GED required. 
  • Relevant experience in human services, mental health, or foster care preferred. 
  • Successful completion of all agency-required screenings and trainings.
